PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for July 15, 2026:
One Stop Nutrition – 2501 W Happy Valley Road, Phoenix
3 violations:
- Oat and almond milk not kept cold enough
- Black organic matter in ice machine
- Blenders not washed correctly
Sushi Mocorrito – 3415 W Glendale Ave, Phoenix
4 violations:
- Raw chicken stored over ready-to-eat sauces
- Dishes with food debris stored as clean
- Cooked rice not held at proper temperature
- Prep cooler not cold enough
Nieman Marcus – 6900 E Camelback Road, Scottsdale
4 violations:
- Chef moved garbage can then grabbed spatula without washing hands
- Dishwasher not cleaning properly
- Cooked beans kept past discard dates
- Cleaning clothes with incorrect sanitizer levels
Loving Hut – 3239 E Indian School Rd, Phoenix
5 violations:
- Employee picked lid off floor, then began touching noodles
- Worker taking ice from container with bare hands
- Employee removed food item from trash using a pot, then put pot back on shelf
- Walk-in cooler not cold enough
- Restaurant using grocery bags to store food
Cyclo Vietnamese Cuisine – 1919 W Chandler Blvd, Chandler
6 violations:
- No certified food manager
- Employee smoking at back kitchen entrance, with smoke going into restaurant
- Worker washing hands without soap
- Employee handling dirty dishes and broom then not washing hands
- No sanitizer in dishwasher
- Box of eggs not held at proper temperature
