PHOENIX — The Arizona Department of Public Safety issued a SAFE Alert Monday afternoon for a missing man last seen in Phoenix.
Carl Bellows, 67, was last seen riding his bike near 36th Street and Broadway Road in Phoenix around 11:30 p.m. Sunday, according to DPS.
He was last seen wearing a gray T-shirt, blue Levi jeans and a light brown jacket with dark brown stripes.

He is 5 feet, 6 inches tall, with gray hair and blue eyes, DPS said.
Bellows has a medical condition that can make him appear confused, according to the alert.
Anybody with information about his whereabouts is asked to call the Phoenix Police Department at 602-534-2121.
SAFE Alerts have replaced Silver Alerts in Arizona as a way to spread the word about missing persons who are 65 and older and missing persons who have dementia or developmental/cognitive disabilities.
