After it enters the tunnel, she appears to nod off until being jolted awake as the train rounds the tunnel and passengers scream as they’re tossed aside. The operator is then seen pressing controls that eventually bring the train to a stop. “I’m sorry, relax,” she could be heard telling passengers as they disembark. “We didn’t crash. Relax. We’re OK. It would have stopped. I don’t know what happened.”KTVU cited the San Francisco Municipal Transportation Agency in reporting that the train reached 50 mph as it rounded the tunnel’s corner, with average Muni speeds typically 8-10 mph. SFMTA said its investigation found “operator fatigue” to blame and that she was put on “non-driving status.” The San Francisco Chronicle reported one passenger said she was treated for a concussion.
