PHOENIX (AZFamily) — Every week, Arizona’s Family looks up restaurants recently inspected by Maricopa County’s Environmental Services Department. Restaurants with some of the highest numbers of “risk factors,” considered major health code violations by inspectors, are selected for our Dirty Dining Report. Here are the Valley restaurants that did not make the grade for May 13, 2026:
Dona Licha Mexican Kitchen – 4344 W Indian School Road, Phoenix
4 violations:
- Employee handled floor mats, then food containers
- No paper towels at hand wash sink
- Cooked beans not cooling properly
- Chili rellenos and lettuce not date marked
Hub Grill and Bar – 1925 S Sossaman Rd, Mesa
4 violations:
- Raw breaded chicken stored on top of raw fish
- Black organic growth in ice machine
- Food debris build-up on meat slicer
- Onion dip kept past discard date
LUCCA Living Room – 2475 W Queen Creek Rd, Queen Creek
4 violations:
- No paper towels at hand wash sink
- Raw chicken stored behind ground beef
- Food debris on can opener blade
- Bolognese sauce not kept at proper temperature
AZ Pho and Grill – 885 N 54th Street, Chandler
5 violations:
- No certified food manager
- Pan of raw beef stored above cooked pork
- Dishwasher not cleaning properly
- Cutting board not washed after cutting shrimp on it
- First aid creams above prep table
Hillside Spot – 4740 E Warner Rd, Phoenix
5 violations:
- Employee came into kitchen, handled dishes without washing hands
- Box of raw bacon over ready-to-eat foods
- Cutting board put away with salsa on it
- Cooked corn stored in water for an hour
- Cream cheese frosting sitting at room temperature
