During his tenure in the U.S. House of Representatives, Collins was a <a href=\"https:\/\/www.govexec.com\/management\/2024\/11\/trump-names-va-pick-firing-and-privatization-come-back-spotlight\/401086\/\" target=\"_blank\" aria-label=\"Link opens in new window (staunch ally)\" rel=\"noopener\">staunch ally<\/a> of the Koch-backed Concerned Veterans for America (CVA), a group whose ideological North Star is privatizing VA services. He embraced three key bills the organization championed that laid the groundwork for outsourcing care for large numbers of VA patients and weakening protections for VA employees.<\/p>\n<p>During his January <a href=\"https:\/\/www.youtube.com\/watch?v=FYxtovXDOBk\" target=\"_blank\" aria-label=\"Link opens in new window (confirmation hearing)\" rel=\"noopener\">confirmation hearing<\/a>, Collins tried his best to shroud his objectives, lulling skeptical committee members with reassuring-sounding platitudes, such as \u201cthere\u2019ll always be a VA for the veteran.\u201d He has continued to spread these unconvincing assertions as VA secretary in interviews, press releases, Twitter videos, and congressional hearings, while, at the same time, swiftly advancing proposals to deeply cut VA resources and personnel and redirect the so-called \u201csavings\u201d toward expanded outsourcing to private-sector care.<\/p>\n<p><a href=\"https:\/\/prospect.org\/topics\/russell-lemle\/\" target=\"_blank\" rel=\"noopener\"><strong>More from Russell Lemle<\/strong><\/a>\u00a0| <a href=\"https:\/\/prospect.org\/topics\/jasper-craven\/\" target=\"_blank\" rel=\"noopener\"><strong>Jasper Craven<\/strong><\/a><\/p>\n<p>His approach has become more combative, marked by disparaging comments directed at reporters, VA employees, and officials responsible for overseeing his agenda. During one recent <a href=\"https:\/\/www.youtube.com\/watch?v=tHi7OWHq3nw\" target=\"_blank\" aria-label=\"Link opens in new window (exchange)\" rel=\"noopener\">exchange<\/a> with Sen. Patty Murray (D-WA), Collins lashed out, \u201cYou\u2019re not being truthful. I\u2019m not going to let you get away with that.\u201d Yet Collins\u2019s stance calls to mind Shakespeare\u2019s famous observation in Hamlet that \u201cThe lady doth protest too much.\u201d It is often the case, after all, that those who most vehemently attack others\u2019 credibility are themselves concealing the truth.<\/p>\n<p>Collins\u2019s privatization campaign rests on four misleading narratives.<\/p>\n<p>First is his pledge that \u201c<a href=\"https:\/\/www.wsbtv.com\/news\/local\/atlanta\/ossoff-presses-secretary-collins-filling-vacancies-atlanta-va-during-budget-hearing\/4GJRZIANW5AOPGQBTYYR3OI7GM\/\" target=\"_blank\" aria-label=\"Link opens in new window (we&#x2019;re not cutting VA health care providers)\" rel=\"noopener\">we\u2019re not cutting VA health care providers<\/a>.\u201d Collins has repeatedly assured stakeholders that, despite his plan to eliminate between 30,000 and 80,000 department positions this year, the number of providers furnishing direct health care won\u2019t decline one bit.<\/p>\n<p>But the VA\u2019s own internal systems contradict these promises. Normally, when VA staff leave, positions are backfilled to maintain care capacity. Presently, however, countless clinical positions remain unfilled as replacement efforts have slowed to a crawl or stopped entirely. Many facilities have simply removed vacated positions from their organizational charts.<\/p>\n<blockquote>\n<p>Since the new administration took office in January, the VA has recorded a net loss of thousands of health care positions.<\/p>\n<p>&#13;\n<\/p><\/blockquote>\n<p>According to the VA\u2019s workforce <a href=\"https:\/\/www.va.gov\/EMPLOYEE\/docs\/workforce\/VA-Workforce-Dashboard-Issue-27.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (dashboards)\" rel=\"noopener\">dashboards<\/a>, since the new administration took office in January the VA has recorded a net loss of thousands of health care positions, the only net loss in the last five years. The losses include, among others, 688 physicians, 1,882 registered nurses, 365 licensed practical nurses, 316 nurse assistants, 511 social workers, 201 psychologists, and 766 medical support assistants. Just in June 2025 alone, VA health system staffing fell by 2,096. Department-wide job announcements have plummeted by 30 percent and applications by 44 percent since last year, while new employee onboarding has sunk by 52 percent. These figures paint a stark picture of an agency diminishing its capacity to provide direct health care, while pretending that its services remain unchanged.<\/p>\n<p>The upshot of this hemorrhaging was illuminated in a recent <a href=\"https:\/\/veterans.house.gov\/calendar\/eventsingle.aspx?EventID=7743\" target=\"_blank\" aria-label=\"Link opens in new window (hearing)\" rel=\"noopener\">hearing<\/a> when Rep. Mariannette Miller-Meeks (R-IA) crowed that \u201cthe higher the ratio between veterans and [VA] staff, the more veterans will need community providers for VA covered specialty care.\u201d In other words, cutting staff has the intended effect of pushing people into the private system.<\/p>\n<p>Second, Collins wrongly portrays private health care as on par with what the VA provides. He has repeatedly asserted that \u201c<a href=\"https:\/\/federalnewsnetwork.com\/workforce\/2025\/06\/va-secretary-says-poor-performing-employees-will-no-longer-fail-up\/\" target=\"_blank\" aria-label=\"Link opens in new window (community care is VA care)\" rel=\"noopener\">community care is VA care<\/a>,\u201d and \u201c<a href=\"https:\/\/www.hydesmith.senate.gov\/hyde-smith-reviews-veterans-affairs-budget-challenges-infrastructure-needs-timely-access-va-care\" target=\"_blank\" aria-label=\"Link opens in new window (we&#x2019;re going to hold community care to the same high standards as VA.)\" rel=\"noopener\">we\u2019re going to hold community care to the same high standards as VA.<\/a>\u201d<\/p>\n<p>The two systems are not the same, however. The Veterans Community Care Program (VCCP) <a href=\"https:\/\/www.military.com\/daily-news\/opinions\/2025\/03\/14\/community-care-not-va-care.html\" target=\"_blank\" aria-label=\"Link opens in new window (lacks critical features)\" rel=\"noopener\">lacks critical features<\/a> that were pioneered at the VA, including integrated care coordination, comprehensive screenings, rigorous inspector general oversight, and public transparency.<\/p>\n<p>A recent Government Accountability Office <a href=\"https:\/\/files.gao.gov\/reports\/GAO-25-106910\/index.html\" target=\"_blank\" aria-label=\"Link opens in new window (report)\" rel=\"noopener\">report<\/a> underscores this disparity: Only 2 percent of VCCP mental health providers have completed even one of the eight veteran-specific trainings required for all VA mental health providers. The sharp contrast highlights how distinct the systems truly are. Attempts by Collins and his allies to blur these differences are part of their justification to divert veterans from the VA to private-sector providers.<\/p>\n<p>Relatedly, Collins often states that \u201c<a href=\"https:\/\/federalnewsnetwork.com\/workforce\/2025\/06\/va-secretary-says-poor-performing-employees-will-no-longer-fail-up\/\" target=\"_blank\" aria-label=\"Link opens in new window (the vast majority of our health care can be done just as effectively outside the VA system)\" rel=\"noopener\">the vast majority of our health care can be done just as effectively outside the VA system<\/a>.\u201d This contention implies that VCCP care quality is comparable to the VA. However, decades of research has established that the VA produces health care outcomes equal or superior to the private sector across virtually every medical condition. A comprehensive <a href=\"https:\/\/veteranspolicy.org\/wp-content\/uploads\/2025\/06\/ReportsSummary.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (summary of peer-reviewed studies)\" rel=\"noopener\">summary of peer-reviewed studies<\/a> published last month reinforces this conclusion. Moreover, a recent survey by Iraq and Afghanistan Veterans of America <a href=\"https:\/\/docs.house.gov\/meetings\/VR\/VR03\/20250715\/118495\/HHRG-119-VR03-Wstate-HunterK-20250715.pdf\" target=\"_blank\" aria-label=\"Link opens in new window (found)\" rel=\"noopener\">found<\/a> that only 31 percent of IAVA members with VCCP experience felt their community providers understand their medical needs.<\/p>\n<p>Finally, Collins claims that \u201c<a href=\"https:\/\/federalnewsnetwork.com\/workforce\/2025\/05\/money-and-people-do-not-solve-the-problems-collins-defends-upcoming-va-workforce-cuts\/\" target=\"_blank\" aria-label=\"Link opens in new window (the VA under the Biden administration stifled veterans getting care in the community)\" rel=\"noopener\">the VA under the Biden administration stifled veterans getting care in the community<\/a>.\u201d While congressional hearings often spotlight anecdotal cases of veterans struggling to access private-sector care, it\u2019s just not true that Biden\u2019s VA cut off community care. During the first three years of his administration, <a href=\"https:\/\/www.usmedicine.com\/agencies\/department-of-veterans-affairs\/congressional-republicans-seek-to-further-expand-vas-use-of-community-care\/\" target=\"_blank\" aria-label=\"Link opens in new window (half a million additional veterans)\" rel=\"noopener\">half a million additional veterans<\/a> gained access to the VCCP.<\/p>\n<p>These misrepresentations carry serious consequences. Community care unquestionably serves as a vital safety net for rural veterans and those facing extended VA wait times. However, last year\u2019s <a href=\"https:\/\/prospect.org\/health\/2024-04-11-privatization-warning-veterans-affairs\/\" target=\"_blank\" rel=\"noopener\">\u201cRed Team\u201d assessment<\/a> by independent experts issued a dire warning: The swelling volume of private-sector referrals has created an \u201cexistential crisis\u201d for VA health care. Excessive utilization of private providers threatens to close VA units and facilities nationwide, destabilizing the direct care system and undermining its long-term sustainability.<\/p>\n<p>Secretary Collins has not been forthright about the adverse consequences of further expanding private-sector care.
