Monroe St. in Jacksonville to pick up a ballot in person.<\/p>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\">Any eligible registered voter can request a vote-by-mail ballot for a specific election or for all elections through the next general election. After the next general election, voters will need to submit a new request if they plan to vote by mail.<\/p>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\">Vote-by-mail ballots may be sent to a voter\u2019s legal residence address, permanent mailing address or a temporary address.<\/p>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\"><b>What voters need to request a ballot by phone<\/b><\/p>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\">To request a vote-by-mail ballot over the phone, voters must provide:<\/p>\n<ul class=\"sc-dLMFU dXsCjt font_openSans articleList\">\n<li class=\"sc-dLMFU sc-eDPEul gFeYkf egxwAv\">\n<p>Full name<\/p>\n<\/li>\n<li class=\"sc-dLMFU sc-eDPEul gFeYkf egxwAv\">\n<p>Date of birth<\/p>\n<\/li>\n<li class=\"sc-dLMFU sc-eDPEul gFeYkf egxwAv\">\n<p>Florida driver\u2019s license or ID number, or the last four digits of their Social Security number \u2014 whichever can be verified in election records<\/p>\n<\/li>\n<li class=\"sc-dLMFU sc-eDPEul gFeYkf egxwAv\">\n<p>Current mailing address, if different from their residential address<\/p>\n<\/li>\n<li class=\"sc-dLMFU sc-eDPEul gFeYkf egxwAv\">\n<p>The election or elections for which the ballot is being requested<\/p>\n<\/li>\n<\/ul>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\"><b>Key deadline<\/b><\/p>\n<p class=\"sc-dLMFU sc-fHjqPf jbiTJv kYFPaL article-text\">Voters should note that ballots cannot be forwarded.
