A six-year-old girl has died of the flu at Northern Beaches hospital after being twice sent home from the Sydney Children\u2019s hospital, and the New South Wales<\/a> health minister has said her parents \u201cdeserve answers\u201d.<\/p>\n

The girl presented to the emergency department at the Sydney Children\u2019s hospital in Randwick on Monday and was discharged after a clinical assessment.<\/p>\n

A spokesperson for the Sydney Children\u2019s Hospitals Network (SCHN) said she presented again \u201cshortly after\u201d and was diagnosed with influenza.<\/p>\n

She was then discharged with at-home guidance, the spokesperson said.<\/p>\n

The girl was later admitted to the Northern Beaches hospital where she died on Tuesday, police confirmed.<\/p>\n

The SCHN spokesperson said upon learning of the girl\u2019s death, \u201cthey expressed their deepest condolences to the family for the loss of their child\u201d.<\/p>\n

The hospital network will be conducting an initial review into the patient\u2019s care while at the Randwick hospital, and will work with the Northern Sydney Local Health<\/a> District as part of the joint independent clinical governance process.<\/p>\n

The NSW health minister, Ryan Park, said: \u201cI\u2019m heartbroken to learn of the death of a child following presentation to Sydney Children\u2019s hospital in Randwick and Northern Beaches hospital.<\/p>\n

\u201cI wish to express my deepest condolences to the family for their devastating loss,\u201d Park said.<\/p>\n

\u201cI want to understand how and why this tragedy has occurred and the parents deserve answers,\u201d Park said. He said he was \u201cmindful how distressing these events are for staff, and I know they\u2019ll be impacted by this tragedy\u201d.<\/p>\n

A spokesperson for the Northern Beaches hospital said: \u201cAlongside Sydney Children\u2019s Hospital Network, where the child received care earlier in the day, we will undertake the appropriate investigations in line with NSW Health policy, including a joint independent review.<\/p>\n

\u201cWhile we cannot comment on the specifics of the case during the review period, our thoughts are with the family, and we will continue to offer them our full support,\u201d they said.<\/p>\n

Park confirmed that the matter has been referred to the coroner and is also the subject of a joint Serious Adverse Event Review.<\/p>\n
