Credit: Shutterstock<\/p>\n<p><strong>Millions of kidney patients could benefit from early detection and prevention as a result of the breakthrough made by scientists at the CRCHUM.<\/strong><\/p>\n<p>In a groundbreaking achievement, researchers at the CRCHUM, the hospital research center affiliated with Universit\u00e9 de Montr\u00e9al, have identified a type of microRNA that can safeguard small blood vessels and help maintain kidney function following severe injury.<\/p>\n<p>This discovery holds significant promise for the more than four million Canadians living with chronic renal failure, as well as millions of patients worldwide, by offering new possibilities for earlier detection and prevention of the disease.<\/p>\n<p>Until now, there had been no dependable biomarker to assess the condition of these delicate capillaries or to guide targeted strategies aimed at protecting kidney function.<\/p>\n<p>Discovery of miR-423-5p as a biomarker<\/p>\n<p>Findings published in JCI Insight reveal that the microRNA known as miR-423-5p shows strong potential as a blood-based biomarker for evaluating kidney microvascular health.<\/p>\n<p>The study was co-authored by Universit\u00e9 de Montr\u00e9al medical professors Marie-Jos\u00e9e H\u00e9bert and H\u00e9lo\u00efse Cardinal, who hold the Shire Chair in Nephrology, Renal Transplantation and Regeneration, alongside H\u00e9bert\u2019s research associate Francis Migneault.<\/p>\n<p><a href=\"https:\/\/scitechdaily.com\/images\/Dr.-Marie-Josee-Hebert-Francis-Migneault-and-Dr.-Heloise-Cardinal-scaled.jpg\" rel=\"nofollow noopener\" target=\"_blank\"><img loading=\"lazy\" decoding=\"async\" class=\"size-large wp-image-491777\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/09\/Dr.-Marie-Josee-Hebert-Francis-Migneault-and-Dr.-Heloise-Cardinal-777x583.jpg\" alt=\"Dr. Marie Jos\u00e9e H\u00e9bert, Francis Migneault, and Dr. H\u00e9lo\u00efse Cardinal\" width=\"777\" height=\"583\"  \/><\/a>Dr. Marie-Jos\u00e9e H\u00e9bert (left) and Dr. H\u00e9lo\u00efse Cardinal (right), CRCHUM researchers and holders of the Shire Chair in Nephrology, Renal Transplantation and Regeneration, co-authored the study with H\u00e9bert\u2019s research associate Francis Migneault (center). Credit: CHUM<\/p>\n<p>Their research focuses on the decline of peritubular capillaries, a key indicator of chronic renal failure.<\/p>\n<p>These minute vessels, found in the kidneys by the millions, are responsible for removing waste from the blood while delivering oxygen and nutrients essential for kidney function.<\/p>\n<p>Risks and potential applications in patients<\/p>\n<p>Kidney damage that occurs when blood flow is temporarily cut off and then restored can result in the loss of small blood vessels, significantly impairing the organ\u2019s ability to function properly.<\/p>\n<p>\u201cIn people who have received a transplant, if kidney function is severely altered, the kidney\u2019s survival is threatened,\u201d said H\u00e9bert, a nephrology-transplant physician and UdeM\u2019s outgoing vice-rector for research, discovery, creation, and innovation.<\/p>\n<p>\u201cUsing this biomarker, a test could be developed to evaluate the status of the small blood vessels much earlier,\u201d she said. \u201cDoctors in hospitals could then better evaluate the microvascular health of higher-risk patients.<\/p>\n<p>\u201cThese could include elderly patients or those undergoing surgeries during which blood flow is temporarily stopped, as is the case for organ transplants or cardiovascular interventions.\u201d<\/p>\n<p>Of mice and\u2026 51 transplant recipients<\/p>\n<p>\u201cWe first observed fluctuating levels of miR-423-5p microRNA in the blood of mice with acute kidney injuries,\u201d said Migneault, the study\u2019s first author. \u201cThese results were then confirmed in 51 transplant recipients who participated in the CHUM kidney transplant biobank.\u201d<\/p>\n<p>Thanks to this biomarker, clinical teams could confirm whether their interventions improve or diminish the health of small blood vessels.<\/p>\n<p>\u201cBut what\u2019s really incredible is that by injecting this microRNA into mice with kidney injuries, we were able to preserve the small blood vessels and limit the damage done to the kidneys,\u201d said Migneault.<\/p>\n<p>While direct injection into the kidney is a clinically feasible method during a transplant, to protect the remaining small blood vessels, the CRCHUM scientists are now focused on alternative techniques to transport the microRNA, or likely a microRNA cocktail, to the kidney.<\/p>\n<p>Potentially useful for other patients<\/p>\n<p>In terms of prevention, a test based on this miR-423-5p microRNA could be useful for patients with cardiac failure, pulmonary failure, or certain neurodegenerative diseases.<\/p>\n<p>\u201cFor these medical conditions, the loss of small blood vessels plays a key role, because of the association with normal or accelerated aging,\u201d said H\u00e9bert. \u201cOur discovery could, therefore, have a significant impact on the health of all Canadians.\u201d<\/p>\n<p>For those with pulmonary failure, several research projects are in progress under Emmanuelle Brochiero, a researcher and head of the Immunopathology research theme at the CRCHUM.<\/p>\n<p>It may also be possible, using the CHUM\u2019s biological material biobank, to determine if existing medications, administered after a kidney transplant to treat another issue, impact small blood vessel health, added H\u00e9bert.<\/p>\n<p>Reference: \u201cEndothelial extracellular vesicle miR-423-5p regulates microvascular homeostasis and renal function after ischemia-reperfusion injury\u201d by Francis Migneault, Hyunyun Kim, Alice Doreille, Shanshan Lan, Alexis Gendron, Marie-H\u00e9l\u00e8ne Normand, Annie Karakeussian Rimbaud, Martin Dupont, Isabelle Bourdeau, \u00c9ric Bonneil, Julie Turgeon, Sylvie Dussault, Pierre Thibault, M\u00e9lanie Dieud\u00e9, \u00c9ric Boilard, Alain Rivard, H\u00e9lo\u00efse Cardinal and Marie-Jos\u00e9e H\u00e9bert, 22 May 2025, JCI Insight.<br \/><a href=\"https:\/\/doi.org\/10.1172\/jci.insight.181937\" rel=\"nofollow noopener\" target=\"_blank\">DOI: 10.1172\/jci.insight.181937<\/a><\/p>\n<p>This research was funded by the Canadian Institutes of Health Research, the Shire Chair in Nephrology, Renal Transplantation and Regeneration at Universit\u00e9 de Montr\u00e9al, the Fondation J.-Louis L\u00e9vesque, the Canadian Donation and Transplantation Research Program, the Fonds de recherche du Qu\u00e9bec\u2014Sant\u00e9, and the Natural Sciences and Engineering Research Council of Canada.
