to have been caused by the Optus triple-0 failure, South Australian police have said.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The baby is <a class=\"Link_link__kR0xA Link_link__5eL5m ScreenReaderOnly_srLinkHint__OysWz Link_showVisited__C1Fea Link_showFocus__ALyv2\" href=\"https:\/\/www.abc.net.au\/news\/2025-09-20\/optus-outage-wa-government-ministers-slam-telco\/105797804\" data-component=\"Link\" data-uri=\"coremedia:\/\/article\/105797804\" rel=\"nofollow noopener\" target=\"_blank\">among four people in WA and SA<\/a> \u2014 including a 68-year-old woman in the Adelaide suburb of Queenstown \u2014 whose deaths were linked to the Optus outage, which caused hundreds of emergency calls to fail.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">SA Police said the investigation into the Queenstown woman&#8217;s death was &#8220;complex&#8221; and that they were &#8220;continuing to determine the impact&#8221; of the outage.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">But police said preliminary investigations had found that while the Gawler West boy&#8217;s family was also impacted by the outage, his death was &#8220;not due to any delay&#8221; in ambulances reaching the home.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;The deceased boy&#8217;s grandmother has told investigating police that she attempted to call triple-0 using her mobile phone when she was alerted to the fact her grandson was not breathing,&#8221; has led to loss of life: analyst<\/a><\/p>\n<p class=\"Typography_base__sj2RP RelatedCard_synopsis__cFwMW Typography_sizeMobile14__u7TGe Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Optus made another catastrophic mistake, but this time three people are dead after the telecommunications giant let customers down &#8220;when they needed them most&#8221;.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The outcome of a full police investigation into the death will be provided to the state coroner, who will determine whether it is, in turn, the subject of an inquest, police said.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">SA Premier Peter Malinauskas said authorities were committed to providing &#8220;appropriate levels of mental health support for that family \u2014 not because of the death relating to Optus, but because that&#8217;s the right thing to do when such a tragedy occurs in any family&#8221;.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">In a statement, the Gawler council said it was &#8220;heartbroken to learn of this child&#8217;s tragic passing&#8221;.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;We offer our deepest and most sincere condolences to the child&#8217;s family during this period of immense grief,&#8221; acting mayor Nathan Shanks said.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Police said while they would not normally comment on individual circumstances, &#8220;the public interest in the matter requires this action&#8221;.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Yesterday, police said they had conducted welfare checks on 154 Optus customers who were affected by the network failure in the state, and that no further adverse outcomes had been reported.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">But Mr Malinauskas stopped short of ruling out further cases coming to light.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">&#8220;We&#8217;re very cautious of accepting everything that Optus says up until this point, and there is still more work to be done in terms of our analysis of their conduct,&#8221;
