More than <a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/dementia\" rel=\"nofollow noopener\" target=\"_blank\">10 million new cases<\/a> of dementia are recorded worldwide each year.<\/p>\n<p>But a study published in 2024 suggests as many as 13 percent of people diagnosed with dementia in the US may actually be misdiagnosed and left struggling with a condition that can be treated.<\/p>\n<p>&#8220;Health care providers [must be] made aware of this potential overlap between dementia and hepatic encephalopathy, which is treatable,&#8221; <a href=\"https:\/\/www.vcuhealth.org\/news\/13-percent-of-patients-with-dementia-may-instead-have-cognitive-decline-from-cirrhosis\" rel=\"nofollow noopener\" target=\"_blank\">said<\/a> Virginia Commonwealth University hepatologist Jasmohan Bajaj in July 2024.<\/p>\n<p><strong>Related: <a href=\"https:\/\/www.sciencealert.com\/millions-of-us-have-likely-lost-our-smell-without-even-realizing-it\" rel=\"nofollow noopener\" target=\"_blank\">Millions of Us Have Likely Lost Our Smell Without Even Realizing It<\/a><\/strong><\/p>\n<p><a href=\"https:\/\/en.wikipedia.org\/wiki\/Hepatic_encephalopathy\" rel=\"nofollow noopener\" target=\"_blank\">Hepatic encephalopathy<\/a> \u2013 cognitive impairment caused by liver failure \u2013 affects more than 40 percent of patients with advanced liver disease (<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/cirrhosis\/symptoms-causes\/syc-20351487\" rel=\"nofollow noopener\" target=\"_blank\">cirrhosis<\/a>). The brain impairments it causes are hard to distinguish from dementia.<\/p>\n<p><a href=\"https:\/\/www.hopkinsmedicine.org\/health\/conditions-and-diseases\/liver-anatomy-and-functions\" rel=\"nofollow noopener\" target=\"_blank\">Our livers regulate<\/a> the levels of most chemicals in our bodies, from filtering them from the blood to aiding digestion.<\/p>\n<p>Beyond <a href=\"https:\/\/www.sciencealert.com\/expert-reveals-what-happens-to-your-liver-when-you-quit-alcohol\" rel=\"nofollow noopener\" target=\"_blank\">alcohol&#8217;s well-known harms<\/a>, other risks to the liver include the hepatitis  <a href=\"https:\/\/www.sciencealert.com\/virus\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73053\" data-postid=\"178109\" rel=\"nofollow noopener\" target=\"_self\">virus<\/a>, high cholesterol, obesity and  <a href=\"https:\/\/www.sciencealert.com\/diabetes\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73018\" data-postid=\"178109\" rel=\"nofollow noopener\" target=\"_self\">diabetes<\/a>, stress, and aging. But if caught early enough and with the right treatment, damage to our livers can be reversed.<\/p>\n<p>Much of this can be mitigated through changes in diet and behavior, and there are even more <a href=\"https:\/\/doi.org\/10.1038\/s43587-024-00652-w\" rel=\"nofollow noopener\" target=\"_blank\">potential drug treatments<\/a> on the horizon.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/10\/AlzheimersSeniorLadyTalkingOnPhone642.jpg\" alt=\"An older woman with grey hair and glasses, talking on the phone\" width=\"642\" height=\"442\" class=\"size-full wp-image-145448\"   loading=\"lazy\"\/>Up to 13 percent of people diagnosed with dementia in the US may have a misdiagnosis. (Rido\/Canva)<\/p>\n<p>In fact, treatment of hepatic encephalopathy has resolved cognitive impairments in <a href=\"https:\/\/doi.org\/10.1016\/j.cgh.2022.04.035\" rel=\"nofollow noopener\" target=\"_blank\">at least two patients<\/a> who had been diagnosed with dementia.<\/p>\n<p>&#8220;He is a different person!&#8221; the wife of one of the patients <a href=\"https:\/\/doi.org\/10.1016\/j.cgh.2022.04.035\" rel=\"nofollow noopener\" target=\"_blank\">notes<\/a>, after his memory loss, falls, tremors, and hallucinations were all resolved.<\/p>\n<p><a href=\"https:\/\/doi.org\/10.1038\/s43587-024-00652-w\" rel=\"nofollow noopener\" target=\"_blank\">A recent study<\/a> in mice suggests that even the impacts of aging on the liver may be reversible if caught early enough.<\/p>\n<p>&#8220;We&#8217;ve shown that aging exacerbates non-alcoholic liver disease\u2026 and by reducing this impact, we can reverse the damage,&#8221; <a href=\"https:\/\/corporate.dukehealth.org\/news\/study-shows-how-liver-damage-stress-and-aging-might-be-reversible\" rel=\"nofollow noopener\" target=\"_blank\">explained<\/a> Duke University hepatologist Anna Mae. &#8220;You are never too old to get better.&#8221;<\/p>\n<p><a href=\"https:\/\/doi.org\/10.1001\/jamanetworkopen.2023.53965\" rel=\"nofollow noopener\" target=\"_blank\">In an earlier 2024 study<\/a>, Bajaj and colleagues reviewed the medical records of 177,422 US veterans diagnosed with dementia between 2009 and 2019.<\/p>\n<p>None of them had been diagnosed with liver disease, but the team found over 10 percent had high fibrosis 4 (FIB-4) scores, an index to measure liver scarring, meaning <a href=\"https:\/\/www.sciencealert.com\/many-dementia-cases-could-actually-be-a-hidden-form-of-liver-disease\" rel=\"nofollow noopener\" target=\"_blank\">they were very likely to have cirrhosis<\/a>.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/10\/StagesOfLiverDamageDiagramMASLD.jpg\" alt=\"diagram showing the different stages of liver damage\" width=\"768\" height=\"615\" class=\"wp-image-132698 size-full\"   loading=\"lazy\"\/>Cirrhosis is the result of persistent liver damage, where healthy tissue has been gradually replaced with scar tissue. (<a href=\"https:\/\/corporate.dukehealth.org\/news\/study-shows-how-liver-damage-stress-and-aging-might-be-reversible\" rel=\"nofollow noopener\" target=\"_blank\">Kuo Du et al., Nature Aging, 2024<\/a>)<\/p>\n<p>In the new study, Virginia Commonwealth University biostatistician Scott Silvey, Bajaj, and colleagues repeated this review with 68,807 medical records of patients from a national database who aren&#8217;t veterans, to see if their earlier results reflected the general US population.<\/p>\n<p>To their surprise, even more patients with high FIB-4 scores were found in this population \u2013 almost 13 percent.<\/p>\n<p><a href=\"https:\/\/www.sciencealert.com\/spark-into-space-comp?utm_source=promo_astro\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/10\/Mid-Article-Promo-Astro-642x272.jpg\" alt=\"Mid Article Promo Astro\" width=\"642\" height=\"272\" class=\"alignnone wp-image-177074 size-medium\"   loading=\"lazy\"\/><\/a><\/p>\n<p>&#8220;The prevalences and determinants of a high FIB-4 are striking, including a greater proportion of patients who were not white in the high FIB-4 group,&#8221; <a href=\"https:\/\/doi.org\/10.1016\/j.amjmed.2024.06.014\" rel=\"nofollow noopener\" target=\"_blank\">explain<\/a> Silvey and team.<\/p>\n<p>&#8220;While we did not study the specific factors behind these disparities, a lack of access to therapy or medical care in both dementia care and comorbidity care could contribute.&#8221;<\/p>\n<p><strong>Related: <a href=\"https:\/\/www.sciencealert.com\/80-of-americans-with-dementia-may-not-even-know-they-have-it\" rel=\"nofollow noopener\" target=\"_blank\">80% of Americans With Dementia May Not Even Know They Have It<\/a><\/strong><\/p>\n<p>Once our livers start to malfunction, it <a href=\"https:\/\/corporate.dukehealth.org\/news\/study-shows-how-liver-damage-stress-and-aging-might-be-reversible\" rel=\"nofollow noopener\" target=\"_blank\">stresses other tissues<\/a> in our bodies, including kidneys, pancreas, and heart, as well as our brains.
