The Bonus Number is used to determine the second prize.<\/p>\n<p>The winner has opted to receive the cash value of the annuitized jackpot in a single lump sum payment of $5,770,232 after required withholdings.<\/p>\n<p>The ticket was purchased at Polge Wine &amp; Liquors located at 434 W.
