{"id":349667,"date":"2025-11-02T05:34:19","date_gmt":"2025-11-02T05:34:19","guid":{"rendered":"https:\/\/www.europesays.com\/us\/349667\/"},"modified":"2025-11-02T05:34:19","modified_gmt":"2025-11-02T05:34:19","slug":"as-texas-applies-for-federal-transformation-funds-rural-hospitals-struggle-to-stay-open","status":"publish","type":"post","link":"https:\/\/www.europesays.com\/us\/349667\/","title":{"rendered":"As Texas applies for federal \u2018transformation\u2019 funds, rural hospitals struggle to stay open"},"content":{"rendered":"<p class=\"body-text-paragraph\">Lots of people have suggestions for how Texas should use its federal rural health funds.<\/p>\n<p class=\"body-text-paragraph\">The <a href=\"https:\/\/www.cms.gov\/priorities\/rural-health-transformation-rht-program\/overview\" rel=\"noopener\" target=\"_blank\">Rural Health Transformation Program<\/a>, created under the \u201cOne Big Beautiful Bill Act,\u201d allocates $50 billion over the next five years for rural health care across the country. Half of those funds will be split evenly amongst the states. States will compete for the remaining half of the funds.<\/p>\n<p class=\"body-text-paragraph\">Texas will soon submit its application, vying against other states for as large of a chunk as it can get of the $25 billion that\u2019s up for grabs. As the state Health and Human Services Commission has worked on the application, the agency has solicited ideas and input from the public.<\/p>\n<p class=\"body-text-paragraph\">Those ideas have been wide-ranging. Representatives of major health systems logged onto the public hearing, too, bringing urban hubs into the rural health conversation.<\/p>\n<p class=\"body-text-paragraph\">While the federal program emphasizes \u201ctransformation,\u201d rural hospital advocates and executive stress that many rural hospitals are fighting simply to keep their doors open.<\/p>\n<p class=\"body-text-paragraph\">\u201cRight now, \u2018sustainability\u2019 and \u2018survival\u2019 would be the words I use before \u2018transformation,\u2019\u201d said Terry Scoggin, the interim CEO and president of the rural hospital advocacy organization TORCH. <\/p>\n<p class=\"body-text-paragraph\">\u201cGet us through two years of this, then let\u2019s start talking about transformation.\u201d<\/p>\n<p class=\"body-text-paragraph\">The state\u2019s rural hospitals have a familiar face to look to in the application process. John Henderson, the CEO of TORCH, has taken a leave of absence from the organization and is working with the state on its application.<\/p>\n<p class=\"body-text-paragraph\">Advocates and CEOs of Texan rural hospitals say they do want to transform rural health care \u2014 they first need enough funding to stabilize their finances, and right a ship that\u2019s constantly threatening to sink.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" style=\"aspect-ratio:4096 \/ 2732\"   class=\"dmnc_images-modern-image-module__QFaG- max-w-full h-auto text-white dmnc_images-modern-image-module__9Zlll bg-gray-light object-contain\" width=\"4096\" height=\"2732\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/S66KPDW5YVFZ7FDCQL2UGM5EEQ.jpg\" alt=\"Dr. Jeremy Johnson performs an ultrasound on Bree Ward, on Friday, Aug. 22, 2025, at Faith...\"\/><\/p>\n<p>Dr. Jeremy Johnson performs an ultrasound on Bree Ward, on Friday, Aug. 22, 2025, at Faith Community Hospital in Jacksboro, TX.<\/p>\n<p>Shafkat Anowar \/ Staff Photographer<\/p>\n<p>At risk of closure<\/p>\n<p class=\"body-text-paragraph\">Many rural hospitals are struggling financially, plagued by a combination of low patient volume and low reimbursement rates.<\/p>\n<p class=\"body-text-paragraph\">\u201cEverybody is struggling right now,\u201d said Rebecca McCain, the CEO of Electra Hospital District. \u201cThat\u2019s where everybody is right now, is trying to survive.\u201d<\/p>\n<p class=\"body-text-paragraph\">About 70% of Texas\u2019 rural hospitals had <a href=\"https:\/\/chqpr.org\/downloads\/Rural_Hospitals_at_Risk_of_Closing.pdf\" rel=\"noopener\" target=\"_blank\">losses on patient services in 2024<\/a>, according to an August report from the Center for Healthcare Quality and Payment Reform. Of the state\u2019s 156 rural hospitals, the same report found, 84 are at risk of closure.<\/p>\n<p class=\"body-text-paragraph\">Mid Coast Medical Center Trinity, about 90 miles north of Houston, announced earlier this year that it was <a href=\"https:\/\/www.texastribune.org\/2025\/04\/23\/texas-rural-trinity-hospital-closes\/\" rel=\"noopener\" target=\"_blank\">closing under financial stress<\/a>. A hospital in Wichita Falls \u2014 a small facility, though not rural \u2014 <a href=\"https:\/\/www.dallasnews.com\/business\/health-care\/2025\/10\/23\/wichita-falls-hospital-closing-its-doors\/\" rel=\"noopener\" target=\"_blank\">announced its closure in October<\/a>.<\/p>\n<p class=\"body-text-paragraph\">For facilities that stay open, the financial strain can still lead to a reduction in services <a href=\"https:\/\/www.dallasnews.com\/business\/health-care\/2025\/09\/18\/half-of-texas-counties-are-maternal-care-deserts-rural-moms-babies-are-paying-the-price\/\" rel=\"noopener\" target=\"_blank\">such as labor and delivery<\/a>, leaving rural residents with less and less access to necessary medical care.<\/p>\n<p class=\"body-text-paragraph\">Rural hospitals are extra on edge now, as they brace for Medicaid cuts enacted by the Trump administration. Nationwide, the federal rural health funds will replace less than 40% of the cuts in Medicaid funding, according to an <a href=\"https:\/\/www.kff.org\/medicaid\/a-closer-look-at-the-50-billion-rural-health-fund-in-the-new-reconciliation-law\/\" target=\"_self\" rel=\"noopener\" title=\"https:\/\/www.kff.org\/medicaid\/a-closer-look-at-the-50-billion-rural-health-fund-in-the-new-reconciliation-law\/\">analysis from the health policy organization KFF<\/a>.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" style=\"aspect-ratio:4096 \/ 2732\"   class=\"dmnc_images-modern-image-module__QFaG- max-w-full h-auto text-white dmnc_images-modern-image-module__9Zlll bg-gray-light object-contain\" width=\"4096\" height=\"2732\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/UAMPXCEBXJBENCVHTPNJIBOFRY.jpg\" alt=\"Dr. Jeremy Johnson walks into the labor and delivery unit in between his ultrasound...\"\/><\/p>\n<p>Dr. Jeremy Johnson walks into the labor and delivery unit in between his ultrasound appointments, on Friday, Aug. 22, 2025, at Faith Community Hospital in Jacksboro, TX.<\/p>\n<p>Shafkat Anowar \/ Staff Photographer<\/p>\n<p class=\"body-text-paragraph\">\u201cThe concern is: Are more hospitals in Texas going to close?\u201d said Stephen Love, the president and CEO of the Dallas-Fort Worth Hospital Council. \u201cAnd\/or, are some of the rural hospitals going to cut services, such as OBGYN?\u201d<\/p>\n<p class=\"body-text-paragraph\">Love said hospital representatives generally agree that innovation \u2014 through new technology, or through pilot programs \u2014 is a good thing.<\/p>\n<p class=\"body-text-paragraph\">\u201cBut it\u2019s not going to solve the immediate, short-term cash flow issues that our rural hospitals face,\u201d he said. \u201cThey need direct financial help to offset the shortfall and the cuts in Medicaid.\u201d<\/p>\n<p class=\"body-text-paragraph\">Facing these headwinds, advocates and CEOs say that rural hospitals need stabilization funds now, so that they can start thinking about transformation and innovation down the road.<\/p>\n<p>Prioritizing rural areas<\/p>\n<p class=\"body-text-paragraph\">Hospital advocates are also keeping an eye on where, exactly, the federal rural health funds go.<\/p>\n<p class=\"body-text-paragraph\">The concern is two-fold. <\/p>\n<p class=\"body-text-paragraph\">First, they worry about how much of the available funds will go to Texas; As a large state with a large population, the structure of the federal program leaves a possibility that Texas could receive fewer dollars per capita than smaller or less populous states.<\/p>\n<p class=\"body-text-paragraph\">\u201cI would hate to see Texas get the same amount of money as, say, Montana,\u201d said Frank Beaman, the CEO of Faith Community Health System in Jacksboro. \u201cNo disrespect towards Montana, but look at how <a href=\"https:\/\/www.ruralhealthinfo.org\/states\/montana\" rel=\"noopener\" target=\"_blank\">many rural hospitals are in Montana<\/a> versus <a href=\"https:\/\/www.torchnet.org\/uploads\/1\/1\/9\/5\/119501126\/torch_10_things_fact_sheet.pdf\" rel=\"noopener\" target=\"_blank\">those that are in Texas<\/a>.\u201d<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" style=\"aspect-ratio:6794 \/ 4529\"   class=\"dmnc_images-modern-image-module__QFaG- max-w-full h-auto text-white dmnc_images-modern-image-module__9Zlll bg-gray-light object-contain\" width=\"6794\" height=\"4529\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/4HZLMABJKNEE3OG47WXSYL6RUM.jpg\" alt=\"Exterior view of Faith Community Hospital on Thursday, May 14, 2020, in Jacksboro, Texas....\"\/><\/p>\n<p>Exterior view of Faith Community Hospital on Thursday, May 14, 2020, in Jacksboro, Texas. Scoggin said he\u2019s concerned by proposals he\u2019s heard from organizations or individuals who are not part of rural Texas.<\/p>\n<p class=\"body-text-paragraph\">\u201c[In] rural communities, we go to church, we go to Walmart, we go to ball games with the same people,\u201d said Scoggin, who was a long-time executive at Titus Regional Medical Center in Mount Pleasant. <\/p>\n<p class=\"body-text-paragraph\">\u201cWe know where the money needs to be spent. We know things that we can do to stabilize.\u201d<\/p>\n<p class=\"body-text-paragraph\">Both Scoggin and McCain said that sending funding to rural hospitals wouldn\u2019t prevent those hospitals from contracting with urban hospitals or institutions, for additional services.<\/p>\n<p class=\"body-text-paragraph\">\u201cOur perspective is if we need that help, we can contract with them for that,\u201d McCain said. \u201cThey\u2019re already there if we need them.\u201d<\/p>\n<p class=\"body-text-paragraph\">Henderson, the rural hospital advocate who\u2019s been working on the state application, expressed a similar perspective in an email, which was included in a batch of public records that the state sent to The News.<\/p>\n<p class=\"body-text-paragraph\">\u201cIf the rural providers see value in your telemedicine service, they can use transformation funds to pay for it,\u201d Henderson wrote to a health system representative who had reached out about the funds, \u201cbut the rurals should control the dollars.\u201d<\/p>\n<p class=\"body-text-paragraph\">The federal applications for the Rural Health Transformation Program are due on Nov. 5.<\/p>\n","protected":false},"excerpt":{"rendered":"Lots of people have suggestions for how Texas should use its federal rural health funds.
