(Supplied: Maersk Shekou&#8217;s P&amp;I representative)<\/p>\n<p>Secondary pilot &#8216;on phone&#8217;<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The report was scathing about the actions of many of the personnel involved in trying to get the ship into the harbour.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Much of the report&#8217;s focus was on the operations of the pilots tasked with navigating the ship into the harbour.<\/p>\n<p><a href=\"https:\/\/www.abc.net.au\/news\/2025-05-19\/sts-leeuwin-ii-repairs-begin-after-fremantle-port-accident\/105309754\" data-component=\"FullBleedLink\" class=\"RelatedCard_link__rsgR9 FullBleedLink_root__lTw_U interactive_focusContext__yRhc_ interactive_defaults__AKxUU FullBleedLink_showVisited__g3Xvz\" target=\"_blank\" rel=\"noopener\">Repairs begin on iconic Leeuwin sailing ship<\/a><\/p>\n<p class=\"Typography_base__sj2RP RelatedCard_synopsis__cFwMW Typography_sizeMobile14__u7TGe Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Nine months after being struck by a container ship at Fremantle Port, the STS Leeuwin II travels south to undergo its restoration.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Day and night, whenever a ship arrives in the waters off Fremantle Port, <a class=\"Link_link__kR0xA Link_link__5eL5m ScreenReaderOnly_srLinkHint__OysWz Link_showVisited__C1Fea Link_showFocus__ALyv2\" href=\"https:\/\/www.abc.net.au\/news\/2019-12-15\/the-people-of-fremantle-port-in-pictures\/11794698\" data-component=\"Link\" data-uri=\"coremedia:\/\/article\/11794698\" target=\"_blank\" rel=\"noopener\">a local team boards the vessel and takes control.<\/a><\/p>\n<p class=\"paragraph_paragraph__iYReA\">Battling wind gusts of up to 54 knots as the container ship approached the port, the report said the pilot failed to tell the helmsman to turn into the inner harbour, meaning the ship continued to barrel towards the Leeuwin.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">And the secondary pilot did not realise the lead pilot had changed course because they were on their mobile phone.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Weather conditions were more severe than had been forecast, with squalls and high winds \u2014 but the unusual conditions were not mentioned when the pilot discussed the ship&#8217;s entry into the harbour with the harbour&#8217;s vessel traffic services operator.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">The Maersk Shekou entered the port while it was still dark \u2014 contrary to harbour rules for ships of that size \u2014 and while weather conditions were too severe for them to safely enter, according to the rules.<\/p>\n<p>Ineffective risk controls<\/p>\n<p class=\"paragraph_paragraph__iYReA\">As part of their probe, investigators conducted four separate simulations of the Maersk Shekou&#8217;s arrival into the harbour using tugboats under similar weather conditions \u2014 and in each case, pilots were able to successfully guide the ship into the port.<\/p>\n<p class=\"paragraph_paragraph__iYReA\">Investigators also highlighted ineffective risk controls by Fremantle Port, which &#8220;likely increased&#8221; safety incidents.<\/p>\n<p><img decoding=\"async\" alt=\"The front of the damaged ship. \" class=\"Image_image__5tFYM ContentImage_image__DQ_cq\"  src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/487a6aa3282cab9726dac1a1d471ae10\" loading=\"lazy\" data-component=\"Image\" data-lazy=\"true\"\/><\/p>\n<p class=\"Typography_base__sj2RP FigureCaption_text__zDxQ5 Typography_sizeMobile12__w_FPC Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Fremantle Port has improved its safety procedures since the Leeuwin accident.
