Surveillance video shared with KCRA 3 showed the frightening incident and the light rail train\u2019s operator appearing fatigued. (See the full video above.)The video begins with the operator slumped at her controls as passengers load for the ill-fated trip. She then leans back in her chair as the vehicle is in motion. After it enters the tunnel, she appears to nod off until being jolted awake as the train rounds the tunnel and passengers scream as they&#8217;re tossed aside.  The operator is then seen pressing controls that eventually bring the train to a stop. \u201cI\u2019m sorry, relax,\u201d she could be heard telling passengers as they disembark. \u201cWe didn\u2019t crash. Relax. We\u2019re OK. It would have stopped. I don\u2019t know what happened.\u201dKTVU cited the San Francisco Municipal Transportation Agency in reporting that the train reached 50 mph as it rounded the tunnel\u2019s corner, with average Muni speeds typically 8-10 mph. SFMTA said its investigation found \u201coperator fatigue\u201d to blame and that she was put on \u201cnon-driving status.\u201d  The San Francisco Chronicle reported one passenger said she was treated for a concussion.
