I don\u2019t know what happened.\u201dKTVU cited the San Francisco Municipal Transportation Agency in reporting that the train reached 50 mph as it rounded the tunnel\u2019s corner, with average Muni speeds typically 8-10 mph. SFMTA said its investigation found \u201coperator fatigue\u201d to blame and that she was put on \u201cnon-driving status.\u201d The San Francisco Chronicle reported one passenger said she was treated for a concussion.
