But this ancient killer is still very much <a href=\"https:\/\/www.nature.com\/articles\/s41467-023-42353-9\" rel=\"nofollow noopener\" target=\"_blank\">a danger in our modern world<\/a>.<\/p>\n<p><a href=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(22)00093-3\/fulltext\" rel=\"nofollow noopener\" target=\"_blank\">Research published in 2022<\/a> revealed the bacterium that causes typhoid fever is evolving extensive drug resistance, and is rapidly replacing strains that aren&#8217;t resistant.<\/p>\n<p>Currently, antibiotics are the only way to effectively treat typhoid, which is caused by the bacterium <a href=\"https:\/\/en.wikipedia.org\/wiki\/Salmonella_enterica\" rel=\"nofollow noopener\" target=\"_blank\">Salmonella enterica<\/a> serovar Typhi (S Typhi). Yet over the past three decades, the bacterium&#8217;s resistance to oral antibiotics has been growing and spreading.<\/p>\n<p><strong>Related: <a href=\"https:\/\/www.sciencealert.com\/powerful-new-antibiotic-was-hiding-in-plain-sight-for-decades\" rel=\"nofollow noopener\" target=\"_blank\">Powerful New Antibiotic Was &#8216;Hiding in Plain Sight&#8217; For Decades<\/a><\/strong><\/p>\n<p>In their study, scientists from multiple countries sequenced the genomes of 3,489 S Typhi strains contracted from 2014 to 2019 in Nepal, Bangladesh, Pakistan, and India, and found a rise in extensively drug-resistant (XDR) Typhi.<\/p>\n<p>Watch the video below for a summary of the study:<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/1763747240_647_0.jpg\" alt=\"YouTube Thumbnail\" tabindex=\"0\" role=\"button\" class=\"youtube-thumbnail-preview\" loading=\"lazy\"\/> frameborder=&#8221;0\u2033 allow=&#8221;accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share&#8221; referrerpolicy=&#8221;strict-origin-when-cross-origin&#8221; allowfullscreen&gt;<\/p>\n<p>XDR Typhi is not only impervious to frontline antibiotics, like ampicillin, chloramphenicol, and trimethoprim\/sulfamethoxazole, but it is also <a href=\"https:\/\/www.coalitionagainsttyphoid.org\/the-issues\/drug-resistant-typhoid\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">growing resistant<\/a> to newer antibiotics, like fluoroquinolones and third-generation cephalosporins.<\/p>\n<p>Even worse, these strains are spreading globally at a rapid rate.<\/p>\n<p>While most XDR Typhi cases stem from south Asia, researchers have identified nearly 200 instances of international spread since 1990.<\/p>\n<p>Most strains have been exported to Southeast Asia, as well as East and Southern Africa, but typhoid superbugs have also been found in the UK, the US, and Canada.<\/p>\n<p>&#8220;The speed at which highly resistant strains of S Typhi have emerged and spread in recent years is a real cause for concern, and highlights the need to urgently expand prevention measures, particularly in countries at greatest risk,&#8221; <a href=\"https:\/\/www.scimex.org\/newsfeed\/the-bacteria-that-cause-typhoid-fever-are-getting-better-at-evading-antibiotics\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">said<\/a> Stanford University infectious disease researcher Jason Andrews when the results were published.<\/p>\n<p><a href=\"https:\/\/www.sciencealert.com\/newsletter?utm_source=promo_health\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/health-newsletter-subscribe-promo-final-642x337.jpg\" alt=\"Subscribe to ScienceAlert's free fact-checked newsletter\" width=\"642\" height=\"337\" class=\"alignnone size-medium wp-image-181305\"   loading=\"lazy\"\/><\/a><\/p>\n<p>Scientists have been warning about drug-resistant typhoid for years now. In 2016, the first XDR typhoid strain was <a href=\"https:\/\/www.who.int\/emergencies\/disease-outbreak-news\/item\/27-december-2018-typhoid-pakistan-en\" rel=\"nofollow noopener\" target=\"_blank\">identified in Pakistan<\/a>. By 2019, it had become the dominant genotype in the nation.<\/p>\n<p>Historically, most XDR typhoid strains have been fought with third-generation antimicrobials, like <a href=\"https:\/\/en.wikipedia.org\/wiki\/Quinolone_antibiotic\" rel=\"nofollow noopener\" target=\"_blank\">quinolones<\/a>, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Cephalosporin\" rel=\"nofollow noopener\" target=\"_blank\">cephalosporins<\/a>, and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Macrolide\" rel=\"nofollow noopener\" target=\"_blank\">macrolides<\/a>.<\/p>\n<p>But by the early 2000s, mutations that confer resistance to quinolones accounted for more than 85 percent of all cases in Bangladesh, India, Pakistan, Nepal, and Singapore. At the same time, cephalosporin resistance was also taking over.<\/p>\n<p>Today, <a href=\"https:\/\/www.coalitionagainsttyphoid.org\/the-issues\/drug-resistant-typhoid\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">only one oral antibiotic is left<\/a>: the macrolide, <a href=\"https:\/\/en.wikipedia.org\/wiki\/Azithromycin\" rel=\"nofollow noopener\" target=\"_blank\">azithromycin<\/a>. And this medicine might not work for much longer.<\/p>\n<p>The 2022 study <a href=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(22)00093-3\/fulltext#seccestitle10\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">found<\/a> mutations that confer resistance to azithromycin are now also spreading, &#8220;threatening the efficacy of all oral antimicrobials for typhoid treatment&#8221;. While these mutations have not yet been adopted by XDR S Typhi, if they are, we are in serious trouble.<\/p>\n<p>If untreated, up to 20 percent of typhoid cases can be fatal, and <a href=\"https:\/\/immunizationdata.who.int\/global\/wiise-detail-page\/typhoid-reported-cases-and-incidence?CODE=Global&amp;YEAR=\" rel=\"nofollow noopener\" target=\"_blank\">in 2024, there were over 13 million cases of typhoid reported<\/a>.<\/p>\n<p>Future outbreaks can be prevented to some extent <a href=\"https:\/\/www.thelancet.com\/journals\/langlo\/article\/PIIS2214-109X(24)00055-X\/fulltext\" rel=\"nofollow noopener\" target=\"_blank\">with typhoid conjugate vaccines<\/a>, but if access to these shots is not expanded globally, the world could soon have another <a href=\"https:\/\/www.sciencealert.com\/worlds-top-infectious-killer-claimed-1-23-million-lives-last-year-who-says\" rel=\"nofollow noopener\" target=\"_blank\">health crisis<\/a> on its hands.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/map_4.20.1-642x361.jpg\" alt=\"A world map highlighting regions where the typhoid vaccine is recommended. (<a href=\"https:\/\/www.cdc.gov\/yellow-book\/hcp\/travel-associated-infections-diseases\/typhoid-and-paratyphoid-fever.html\" rel=\"nofollow noopener\" target=\"_blank\">CDC<\/a>)<\/p>\n<p>&#8220;The recent emergence of XDR and azithromycin-resistant S Typhi creates greater urgency for rapidly expanding prevention measures, including use of typhoid conjugate vaccines in typhoid-endemic countries,&#8221; the authors <a href=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(22)00093-3\/fulltext#seccestitle10\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">write<\/a>.<\/p>\n<p>&#8220;Such measures are needed in countries where antimicrobial resistance prevalence among S Typhi isolates is currently high, but given the propensity for international spread, should not be restricted to such settings.&#8221;<\/p>\n<p><strong>Related: <a href=\"https:\/\/www.sciencealert.com\/tomato-juice-can-kill-salmonella-the-bacteria-that-terrorizes-our-guts\" rel=\"nofollow noopener\" target=\"_blank\">Tomato Juice Can Kill Salmonella, The Bacteria That Terrorizes Our Guts<\/a><\/strong><\/p>\n<p>South Asia might be the main hub for typhoid fever, <a href=\"https:\/\/doi.org\/10.1016\/j.vaccine.2025.127848\" rel=\"nofollow noopener\" target=\"_blank\">accounting for 70 percent<\/a> of all cases, but if  <a href=\"https:\/\/www.sciencealert.com\/coronavirus\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73041\" data-postid=\"182049\" rel=\"nofollow noopener\" target=\"_self\">COVID-19<\/a> taught us anything, it is that <a href=\"https:\/\/www.sciencealert.com\/mysterious-driving-factor-behind-long-covid-may-have-been-identified\" rel=\"nofollow noopener\" target=\"_blank\">disease variants<\/a> in <a href=\"https:\/\/www.sciencealert.com\/the-coronavirus-spawns-new-variants-with-alarming-speed-and-we-may-finally-know-how\" rel=\"nofollow noopener\" target=\"_blank\">our modern, globalized world are easily spread<\/a>.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/11\/Typhoid_inoculation.jpg\" alt=\"Typhoid inoculation\" width=\"642\" height=\"470\" class=\"wp-image-146182 size-full\"   loading=\"lazy\"\/>Typhoid inoculation at a rural school, San Augustine County, Texas. (<a href=\"https:\/\/commons.wikimedia.org\/wiki\/File:Typhoid_inoculation2.jpg\" rel=\"nofollow noopener\" target=\"_blank\">John Vachon\/United States Farm Security Administration\/Wikimedia Commons\/Public Domain<\/a>)<\/p>\n<p>To prevent that from happening, health experts argue nations must expand access to typhoid vaccines and invest in <a href=\"https:\/\/www.sciencealert.com\/newly-discovered-antibiotic-could-fight-one-of-the-nastiest-superbugs\" rel=\"nofollow noopener\" target=\"_blank\">new antibiotic research<\/a>.<\/p>\n<p>A 2021 <a href=\"https:\/\/doi.org\/10.1016\/j.vaccine.2021.06.003\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">study<\/a> in India, for instance, estimated that if children are vaccinated against typhoid in urban areas, it could prevent up to 36 percent of typhoid cases and deaths.<\/p>\n<p>Pakistan is leading the way on this front. It was the first nation in the world to offer routine <a href=\"https:\/\/www.cdc.gov\/vaccines\/hcp\/current-vis\/typhoid.html\" rel=\"nofollow noopener\" target=\"_blank\">immunization for typhoid<\/a>. A small number of countries have now followed suit or are &#8220;<a href=\"https:\/\/doi.org\/10.1016\/j.vaccine.2025.127848\" rel=\"nofollow noopener\" target=\"_blank\">planning or considering introduction<\/a>&#8220;.<\/p>\n<p>The  <a href=\"https:\/\/www.sciencealert.com\/what-does-the-world-health-organisation-do\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73079\" data-postid=\"182049\" rel=\"nofollow noopener\" target=\"_self\">World Health Organization<\/a> has prequalified four typhoid conjugate vaccines as of April 2025, <a href=\"https:\/\/doi.org\/10.1016\/j.vaccine.2025.127848\" rel=\"nofollow noopener\" target=\"_blank\">according to the CDC<\/a>, which states &#8220;these vaccines are being introduced into childhood immunization programs in typhoid-endemic countries.&#8221;<\/p>\n<p>Antibiotic resistance is one of <a href=\"https:\/\/www.sciencealert.com\/the-third-leading-cause-of-death-globally-in-2019-was-antibiotic-resistant-bacterial-infection\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">the world&#8217;s leading causes of death<\/a>, claiming the lives of more people than  <a href=\"https:\/\/www.sciencealert.com\/hiv\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73049\" data-postid=\"182049\" rel=\"nofollow noopener\" target=\"_self\">HIV<\/a>\/ <a href=\"https:\/\/www.sciencealert.com\/hiv\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73049\" data-postid=\"182049\" rel=\"nofollow noopener\" target=\"_self\">AIDS<\/a> or  <a href=\"https:\/\/www.sciencealert.com\/malaria\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73073\" data-postid=\"182049\" rel=\"nofollow noopener\" target=\"_self\">malaria<\/a>. <a href=\"https:\/\/www.sciencealert.com\/vaccines-have-saved-a-staggering-154-million-lives-in-the-last-50-years\" rel=\"nofollow noopener\" target=\"_blank\">Where available, vaccines are<\/a> <a href=\"https:\/\/www.thelancet.com\/journals\/langlo\/article\/PIIS2214-109X(22)00210-8\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">some of the best tools<\/a> we have to <a href=\"https:\/\/www.sciencealert.com\/emergency-vaccines-reduce-disease-outbreak-deaths-by-almost-60\" rel=\"nofollow noopener\" target=\"_blank\">prevent future catastrophe<\/a>.<\/p>\n<p>We don&#8217;t have time to waste.<\/p>\n<p>The study was published in <a href=\"https:\/\/www.thelancet.com\/journals\/lanmic\/article\/PIIS2666-5247(22)00093-3\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">The Lancet Microbe<\/a>.<\/p>\n<p>An earlier version of this article was published in June 2022.<\/p>\n","protected":false},"excerpt":{"rendered":"Despite having plagued humans for millennia, typhoid fever is rarely considered a threat in developed countries today.
