Other state hospital groups are struggling."},"content":{"rendered":"<p class=\"paragraph | gutter_20_0\">\u201cThe layoffs may well have been appropriate,\u201d said Dr. John Freedman, CEO of Freedman HealthCare, a Massachusetts-based national health policy consulting firm. \u201cEven though they are profitable, they may have needed them. Sometimes it\u2019s good to organize. But it does call into question the alarm they shared at the time that they had to do that or they would be in big trouble.\u201d<\/p>\n<p class=\"paragraph | gutter_20_0\">Friday\u2019s financial report not only <a href=\"https:\/\/www.bostonglobe.com\/2025\/11\/28\/metro\/snap-food-stamps-employers-massachusetts\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/11\/28\/metro\/snap-food-stamps-employers-massachusetts\/?p1=BGSearch_Overlay_Results\">shows the maneuverability of the state\u2019s largest employer<\/a>, but stands in contrast to some other Massachusetts hospital groups, <a href=\"https:\/\/www.bostonglobe.com\/2025\/12\/19\/business\/tufts-umass-hospitals-cuts-operating-losses\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/12\/19\/business\/tufts-umass-hospitals-cuts-operating-losses\/\">several of whom reported multi-million dollar operating losses in the last year. <\/a><\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cMGB (is) spending a lot of money on building and investing in stuff despite (headwinds) and are able to show a robust bottom line, which is incredibly impressive,\u201d Freedman said. \u201cI wish that were able to be true for the rest of our providers.\u201d <\/p>\n<p><img decoding=\"async\" id=\"img-3EC734INMYY2FO37E336OW7D34-image\" alt=\"Construction last year on two new patient towers at Massachusetts General Hospital.\" class=\"height_a width_full invisible width_full--mobile width_full--tablet-only\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/12\/3EC734INMYY2FO37E336OW7D34.jpg\"  loading=\"lazy\"\/>Construction last year on two new patient towers at Massachusetts General Hospital.David L. Ryan\/Globe Staff<\/p>\n<p class=\"paragraph | gutter_20_0\">Revenue from <a href=\"https:\/\/www.bostonglobe.com\/2025\/12\/03\/business\/mass-general-brigham-burn-unit\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/12\/03\/business\/mass-general-brigham-burn-unit\/?p1=BGSearch_Overlay_Results\">patient activities grew nearly 8 percent<\/a> this year to $14.5 billion, as the hospital saw more patients and reduced the length of time each patient was in the hospital. Additionally, the system grew health plan revenue and <a href=\"https:\/\/www.bostonglobe.com\/2025\/11\/06\/nation\/alzheimers-disease-study-says-walking-can-slow-progression\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/11\/06\/nation\/alzheimers-disease-study-says-walking-can-slow-progression\/?p1=BGSearch_Overlay_Results\">diversified other areas of revenue growth<\/a>, <a href=\"https:\/\/www.bostonglobe.com\/2025\/09\/23\/business\/mass-general-brigham-primary-care\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/09\/23\/business\/mass-general-brigham-primary-care\/?p1=BGSearch_Overlay_Results\">contributing to a $22.8 billion operating revenue<\/a>. <\/p>\n<p class=\"paragraph | gutter_20_0\"><a href=\"https:\/\/www.bostonglobe.com\/2025\/11\/24\/business\/mass-general-brigham-beth-israel-lahey\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/11\/24\/business\/mass-general-brigham-beth-israel-lahey\/?p1=BGSearch_Overlay_Results\">Employee compensation and benefits expenses<\/a> also grew 8 percent \u2014 which included the cost of restructuring, alongside higher pharmaceutical expenses, contributing to an operating expense of $22.7 billion. <\/p>\n<p class=\"paragraph | gutter_20_0\">Mass General Brigham declined to make an executive available to talk about the results, but chief financial officer Niyum Gandhi said in a statement that the organization would continue to invest in innovation, patient care and community health. <\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cHealthcare is transforming \u2014 and Mass General Brigham is helping define what\u2019s next,\u201d Gandhi said. \u201cIn 2025, our system advanced care, strengthened research, and invested in its people, all while navigating complexity across healthcare.\u201d<\/p>\n<p class=\"paragraph | gutter_20_0\">The organization downplayed its non-operating margins, saying the gains and losses on investments vary due to the volatility in the market. While the system said it relies on operating revenue to pay for things like staff, equipment, maintenance and medical supplies, investment returns typically fund capital projects and major renovations. <\/p>\n<p class=\"paragraph | gutter_20_0\"><a href=\"https:\/\/www.bostonglobe.com\/2025\/10\/01\/business\/mass-general-new-balance-foundation\/?p1=BGSearch_Overlay_Results\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/10\/01\/business\/mass-general-new-balance-foundation\/?p1=BGSearch_Overlay_Results\">There is no shortage of those<\/a>. The system has <a href=\"https:\/\/www.bostonglobe.com\/2021\/04\/22\/business\/mgh-pushes-ahead-with-nearly-2-billion-expansion\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2021\/04\/22\/business\/mgh-pushes-ahead-with-nearly-2-billion-expansion\/\">undertaken a $2 billion expansion of Mass General Hospital<\/a>, which is expected to <a href=\"https:\/\/www.bostonglobe.com\/2025\/09\/30\/business\/brigham-womens-new-tower\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/09\/30\/business\/brigham-womens-new-tower\/\">open in 2027<\/a>, and announced this year <a href=\"https:\/\/www.bostonglobe.com\/2025\/09\/30\/business\/brigham-womens-new-tower\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/09\/30\/business\/brigham-womens-new-tower\/\">it would replace the main building<\/a> at Brigham and Women\u2019s Hospital. <\/p>\n<p class=\"paragraph | gutter_20_0\">While this year\u2019s $1.8 billion in investment income may largely exist on paper, Freedman said he viewed those gains as identical to earnings, as investments can be liquidated and used for whatever the health system wants. <\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cI think it\u2019s disingenuous when they minimize it when the numbers go up,\u201d Freedman said. \u201cWhen the numbers go down they say, \u2018oh my God, we got slaughtered and look how much money we lost.\u2019 They can\u2019t have it both ways. They have had two years of outrageously positive returns. They are able to generate them because they have enormous reserves that no other provider has. Some other providers can\u2019t even come anywhere close. That\u2019s not really fair. That counts.\u201d<\/p>\n<p class=\"paragraph | gutter_20_0\">The different finances and outlooks of the hospitals in the state are mirrored nationwide, said John McDonough, <a href=\"https:\/\/hsph.harvard.edu\/profile\/john-e-mcdonough\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/hsph.harvard.edu\/profile\/john-e-mcdonough\/\">a professor at the Harvard T.H. Chan School of Public Health. <\/a><\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cWe\u2019ve got two definite tiers,\u201d McDonough said. \u201cThe high fliers \u2014 what some people call the mega providers \u2014 who are doing in general pretty well. And the others \u2014 the independents, safety nets, rural (hospitals), who are more than struggling. They are, in some cases, desperate.\u201d<\/p>\n<p class=\"paragraph | gutter_20_0\">Those differences will dictate how providers fare in the years ahead. Changes to subsidies for health insurance <a href=\"https:\/\/www.bostonglobe.com\/2025\/12\/09\/business\/massachusetts-health-insurance-connector\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/12\/09\/business\/massachusetts-health-insurance-connector\/\">may increase the number of uninsured<\/a> individuals in Massachusetts in the years ahead, pressuring providers. Medicaid cuts <a href=\"https:\/\/www.bostonglobe.com\/2025\/07\/09\/metro\/medicaid-mashealth-trump-massachusetts-budget\/\" target=\"_blank\" rel=\"noopener\" title=\"https:\/\/www.bostonglobe.com\/2025\/07\/09\/metro\/medicaid-mashealth-trump-massachusetts-budget\/\">may present further challenges<\/a> further down the line. <\/p>\n<p class=\"paragraph | gutter_20_0\">Some poorer systems may not survive, McDonough said. For those who do, it may be a bumpy ride, but one with immense promise. <\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cWhenever a rocky period like this happens, there is a shakedown,\u201d he said. \u201cIt\u2019s tough for everybody to get through. The ones that do will find life to be even better on the other side. The rich get richer, and the poor get poorer.\u201d<\/p>\n<p class=\"paragraph | gutter_20_0\">Paul Hattis, a senior fellow at the think tank the Lown Institute, said part of how the rich stay rich is staying ahead of anticipated changes. He pointed to MGB\u2019s layoffs as a proactive step amid a tighter financial outlook. <\/p>\n<p class=\"paragraph | gutter_20_0\">But Freedman noted that Mass General Brigham will likely face less pressure than some other providers from Medicaid cuts and growing ranks of uninsured patients. And as hospital providers writ large enter a period of uncertainty, MGB is in a considerably stronger position. <\/p>\n<p class=\"paragraph | gutter_20_0\">\u201cIt\u2019s a good thing for the Commonwealth they are doing well, don\u2019t get me wrong,\u201d Freedman said. \u201cBut I\u2019m worried about the others.
