When you\u2019re at the gym, most treadmills are set to a steep slope as folks periodically tap the incline button.<\/p>\n<p>Social media is also a big fan of the incline. You\u2019ve got the <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.outsideonline.com\/health\/training-performance\/12-3-30-workout\/\">12-3-30 treadmill workout<\/a> (which involves setting the incline to 12 percent and walking at three miles per hour for 30 minutes) that went viral after influencer <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.instagram.com\/laurengiraldo\/?hl=en\">Lauren Giraldo<\/a> said it helped her reach her fitness goals. Then there\u2019s the popular <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.popsugar.com\/fitness\/hot-girl-hike-editor-experiment-49472483\">\u201chot girl hikes,\u201d<\/a> founded by influencer <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.hotgirlhikes.com\/about\">Katie Gassman<\/a>, in which \u201chot girl hosts\u201d lead trips to major national parks.<\/p>\n<p>But long before trending workouts and influencer-inspired excursions, walking uphill served many functions: a way to fight disease, punish inmates, prepare military personnel for battle, and gather real-time data on heart health.<\/p>\n<p>Tuberculosis and Uphill Walking as a Prescription<\/p>\n<p>Tuberculosis (TB), one of the world\u2019s oldest and most persistent diseases, has plagued humanity for over <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5432783\/\">70,000 years<\/a>. Just in 2024 alone, it was reported that <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.pbs.org\/newshour\/health\/global-tuberculosis-case-rise-to-a-record-but-deaths-fall-reports-who\">over eight million people<\/a> around the world received a TB diagnosis.<\/p>\n<p>But before antibiotics arrived in the 20th century, with the <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.acs.org\/education\/whatischemistry\/landmarks\/flemingpenicillin.html\">discovery of penicillin<\/a>, the medical response to a TB case was blunt but deliberate: <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/wellcomecollection.org\/stories\/the-history-of-sanatoriums-and-surveillance\">fresh air, high altitude, meticulous hygiene<\/a>, and <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/upload.wikimedia.org\/wikipedia\/commons\/1\/1c\/The_diagnosis_and_modern_treatment_of_pulmonary_consumption%2C_with_special_reference_to_the_early_recognition_and_the_permanent_arrest_of_the_disease_%28IA_cu31924000319479%29.pdf\">uphill walking<\/a>. These prescriptive measures took place in <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9624135\/\">isolated<\/a> medical facilities known as sanatoriums, and some of the fancier ones resembled a cross between a hospital and a resort.<\/p>\n<p><img alt=\"sanatorium\" loading=\"lazy\" width=\"2400\" height=\"1350\" decoding=\"async\" data-nimg=\"1\" class=\"alignnone size-full wp-image-2730627\" style=\"color:transparent\"  src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2026\/02\/sanatorium-in-copy-photo.jpg\"\/>The vastness of a sanatorium. Source: Wellcome Collection.)<\/p>\n<p>Among the first to formalize uphill walking as a prescription was <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5432783\/\">Dr. Hermann Brehmer<\/a>, who, in 1854,<strong>\u00a0<\/strong>claimed that TB could be cured in high-altitude locations, based on his own recovery from the disease after a stint in the Himalayas.<\/p>\n<p>The reason? Uphill walking at a low speed\u2014Brehmer insisted on no more than <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/upload.wikimedia.org\/wikipedia\/commons\/1\/1c\/The_diagnosis_and_modern_treatment_of_pulmonary_consumption%2C_with_special_reference_to_the_early_recognition_and_the_permanent_arrest_of_the_disease_%28IA_cu31924000319479%29.pdf\">two to three miles per hour<\/a>\u2014improved heart function.<\/p>\n<p>Traversing Uneven Terrain in the Military<\/p>\n<p>Fast forwarding to the 1940s, the United States military had codified a simple truth: Fitness began with movement, not machinery.<\/p>\n<p>The military <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.ibiblio.org\/hyperwar\/USA\/ref\/FM\/PDFs\/FM21-20.PDF?\">FM 21-20 Basic Field Manual,<\/a> issued in 1941, laid the foundation for combat conditioning and explored the physiological benefits of \u201ctraversing rough and varied terrain.\u201d Marching up and down irregular ground, according to the manual, was \u201cdesigned to develop endurance, agility, and coordination.\u201d<\/p>\n<p>The Treadmill Era<\/p>\n<p>In the early 19th century, treadmills existed, but not in the way we know them today. An engineer named <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/daily.jstor.org\/treadmills-were-meant-to-be-atonement-machines\/\">William Cubitt<\/a> invented the earlier iteration of the treadmill, called a treadwheel. Rather than being purely used for exercise, they were <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.bbc.co.uk\/programmes\/articles\/4w8bVrKRqQDP4fKl0b8XzdW\/the-dark-history-of-the-treadmill\">instruments of moral correction<\/a> designed to wear prisoners down through endless exertion.<\/p>\n<p><img alt=\"older iteration of the treadmill as prisoner punishment\" loading=\"lazy\" width=\"2400\" height=\"1350\" decoding=\"async\" data-nimg=\"1\" class=\"alignnone size-full wp-image-2730635\" style=\"color:transparent\"  src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2026\/02\/treadmill-punishment-final-1.jpg\"\/>A treadmill in a London prison in 1817. The wheel could then be attached to a corn-grinding machine so that each step would help mash the kernels. But what was once punitive would eventually be relabeled.<\/p>\n<p>The turning point came in the <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/01.cir.0000164274.41137.75\">1960s<\/a>, led by cardiologist Dr. Robert A. Bruce. He recognized the opportunity to <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/bcmj.org\/articles\/stress-testing-contribution-dr-robert-bruce-father-exercise-cardiology\">use the treadmill<\/a> to help diagnose heart disease with a method known as the Bruce Protocol. Today, it\u2019s now one of the most trusted methods in cardiology.<\/p>\n<p>The test is simple. The patient walks at a low speed, just 1.7 miles per hour, at a ten percent incline. Every three minutes, both metrics tick upward, eventually reaching 5.5 mph at a 22 percent incline. Patients walk until they are fatigued or express a desire to stop the test. Meanwhile, clinicians look for symptoms of <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/21489-angina\">angina<\/a> (intense chest pain) and <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/my.clevelandclinic.org\/health\/symptoms\/16942-dyspnea\">dyspnea<\/a>, which refers to breathing difficulties. They\u2019ll also <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK499903\/\">log patients<\/a>\u2019 heart rate, blood pressure, and electrical signals in the heart.<\/p>\n<p>Bruce\u2019s protocol was used because it was a safe, repeatable way to test heart health and function. And so incline walking ultimately became a window into cardiovascular resilience.<\/p>\n<p>The Modern Rebrand of Incline Walking and Why We Keep Coming Back to It<\/p>\n<p>Like most fitness trends, the practice of incline walking has persisted, but its branding has evolved. The 12-3-30 workout repackages old logic into a sleek formula. It\u2019s marketed as transformative\u2014and for many, it may be. But its effectiveness isn\u2019t new. It\u2019s the same uphill approach\u00a0used in sanatoriums, military field manuals, and cardiac stress labs for over a century. Why? Walking on an incline, <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11624387\/\">according to research<\/a>, engages the lower body and may improve cardiovascular health without overstressing the joints, which is especially beneficial for older adults. The appeal? Low impact, high return.<\/p>\n<p>Even mountaineering training <a target=\"_blank\" class=\"text-brand-primary underline hover:text-brand-primary\/85 break-words overflow-wrap-anywhere underline-offset-[3px]\" rel=\"noopener nofollow\" data-afl-p=\"0\" href=\"https:\/\/www.rei.com\/learn\/expert-advice\/mountaineering-how-to-cross-train.html\">guidance<\/a> includes step-up exercises to help prime the glutes and quads for intense uphill climbs with a load on your back. And the previously mentioned hot girl hikes make incline walking trendy, fun, and community-driven.<\/p>\n<p>Uphill walking has worn multiple hats. But the method hasn\u2019t changed\u2014just the marketing. What was once therapy, protocol, or punishment is now a lifestyle.<\/p>\n<p>Incline walking, or walking in general,\u00a0 still seems to get treated like a placeholder\u2014something to do between more intense workouts like weightlifting or running. But it keeps showing up where it counts. Maybe that\u2019s why it resurfaces under new names, new hashtags, and viral formats.<\/p>\n<p>Walking returns not because it\u2019s flashy, but because it\u2019s effective.
