Restaurants with some of the highest numbers of \u201crisk factors,\u201d considered major health code violations by inspectors, are selected for our Dirty Dining Report. Hampton Ave., Mesa <\/p>\n<p class=\"text | article-text\">4 violations: <\/p>\n<ul class=\"list | article-list\">\n<li>Employee re-used plastic glove stored in raw beef<\/li>\n<li>Box of jalapenos had organic debris<\/li>\n<li>Old meat debris on deli slicer<\/li>\n<li>Pork and brisket not kept cold enough<\/li>\n<\/ul>\n<p class=\"text | article-text\"><b>Light Heart Coffee<\/b> &#8211; 51 W.
