Public domain photo<\/p>\n<p>Aging services advocates say the FY 2026 budget bill, which includes <a href=\"https:\/\/ccf.georgetown.edu\/2025\/06\/26\/one-big-beautiful-bill-act-winners-and-losers-in-the-medicaid-provisions\/\" target=\"_blank\" rel=\"noreferrer noopener\">massive cuts to Medicaid<\/a> and <a href=\"https:\/\/ffis.org\/budget-brief\/one-big-beautiful-bill-act\/\" target=\"_blank\" rel=\"noreferrer noopener\">nutrition services<\/a>, will mean the <a href=\"https:\/\/www.news10.com\/news\/ny-news\/federal-budget-impact-new-york\/\" target=\"_blank\" rel=\"noreferrer noopener\">upheaval of vital programs<\/a> keeping older adults healthy and aging in their communities.\u00a0<\/p>\n<p>Journalists can use any of the following tips as a starting point to ask local, state, and federal policymakers how these cuts will affect their older constituents, how the state budget will address the shortfalls and what might happen to the millions of older adults in their communities.<\/p>\n<p>Medicaid is a joint federal-state program. <a href=\"https:\/\/www.medicaid.gov\/medicaid\/benefits\/mandatory-optional-medicaid-benefits\" target=\"_blank\" rel=\"noreferrer noopener\">It pays for<\/a> many services for <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/congressional-district-interactive-map-medicaid-enrollment-by-eligibility-group\/\" target=\"_blank\" rel=\"noreferrer noopener\">about 9 million older adults<\/a> \u2014 like home care assistants, adult day care and transportation to medical appointments \u2014 that are not covered by Medicare. It also provides much-needed financial assistance for health care costs, prescription drugs, dental, vision and hearing care. But with significant Medicaid cuts on the horizon, states will have to make up the shortfall somewhere. Advocacy groups say many programs will be significantly scaled back or eliminated, leaving older adults and their families to figure out how to pay for care. The <a href=\"https:\/\/d1dth6e84htgma.cloudfront.net\/E_and_C_Markup_Subtitle_D_Part_I_5_12_25_4628d60c2a.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Congressional Budget Office estimates<\/a> that about 1.3 million people over 65 will lose their Medicaid coverage under the budget plan by 2034.<\/p>\n<p><strong>Among the programs and services affected<\/strong><\/p>\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/www.medicareinteractive.org\/understanding-medicare\/cost-saving-programs\/medicare-savings-programs-qmb-slmb-qi\/medicare-savings-program-basics\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Medicare Shared Savings Program<\/strong><\/a>: This program helps dual-eligibles \u2014 low-income older adults who receive both Medicare and Medicaid \u2014 pay for their <a href=\"https:\/\/www.kff.org\/medicaid\/report\/nursing-facilities-staffing-residents-and-facility-deficiencies-2009-through-2016\/\" target=\"_blank\" rel=\"noreferrer noopener\">Medicare premiums<\/a>. The <a href=\"https:\/\/www.cbo.gov\/system\/files\/2025-05\/Wyden-Pallone_Letter.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Congressional Budget Office estimates<\/a> that 1.4 million Medicare enrollees could lose this financial assistance. The Medicare Rights Center says that a restructuring of Medicaid financing could eliminate many home and community-based services that <a href=\"https:\/\/www.medicaid.gov\/medicaid\/benefits\/mandatory-optional-medicaid-benefits\" target=\"_blank\" rel=\"noreferrer noopener\">aren\u2019t mandated <\/a>under existing regulations.<\/li>\n<li><strong>Home and community-based services<\/strong>: These help older people with activities of daily living, like bathing, dressing and toileting. They\u2019re important services that help keep people out of nursing homes. But because these programs are not mandatory, they will likely be a key target for cuts, <a href=\"https:\/\/www.milbank.org\/2025\/03\/aging-at-risk-the-impact-of-medicaid-cuts-on-older-adults\/\" target=\"_blank\" rel=\"noreferrer noopener\">The Scan Foundation\u2019s<\/a> Sarita Mohanty, M.D., and Narda Ipakchi wrote. \u201cStates with fewer federal dollars for Medicaid might restrict access for HCBS by modifying eligibility standards or reducing benefits. Ironically, this could ultimately end up costing states more, as HCBS has been <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22106902\/#:~:text=Abstract,term%20care%20(LTC)%20programs.\" target=\"_blank\" rel=\"noreferrer noopener\">shown to be cost-effective<\/a> by delaying or preventing the need for more expensive institutional care,\u201d they said.<\/li>\n<li><strong>Direct Care Workforce<\/strong>: Medicaid recipients who need home-based services are required to use Medicaid-certified agencies. However, funding cuts will <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/payment-rates-for-medicaid-home-and-community-based-services-states-responses-to-workforce-challenges\/\" target=\"_blank\" rel=\"noreferrer noopener\">reduce payments to these agencies<\/a>, where workforce <a href=\"https:\/\/www.phinational.org\/resource\/direct-care-workers-in-the-united-states-key-facts-2024\/\" target=\"_blank\" rel=\"noreferrer noopener\">recruitment and retention<\/a> are already a major problem, according to the direct care workforce research and <a href=\"https:\/\/www.phinational.org\/about\/#:~:text=PHI%20works%20to%20transform%20eldercare,the%20foundation%20for%20quality%20care.\" target=\"_blank\" rel=\"noreferrer noopener\">advocacy organization PHI<\/a>. It will become harder to find workers or agencies to provide vital home care.<\/li>\n<\/ul>\n<p>Medicaid is not the only target of the budget. Food assistance \u2014 SNAP, the Supplemental Nutrition Assistance Program run by the USDA, will see about $300 billion in cuts, or about 30% of its funding over the next decade, <a href=\"https:\/\/www.healthaffairs.org\/content\/forefront\/house-snap-cuts-would-further-endanger-medicaid-disabled-people-older-adults\" target=\"_blank\" rel=\"noreferrer noopener\">according to Health Affairs<\/a>.\u00a0<\/p>\n<p>While Republican lawmakers have consistently said that qualified elderly, children and those with disabilities would not lose benefits, elder advocates noted,\u00a0 \u201cSNAP is a household-based program, when able-bodied adults in the household lose benefits due to work requirements, SNAP allotments would be cut for the remaining members, who <a href=\"https:\/\/www.fns.usda.gov\/research\/snap\/characteristics-fy23\" target=\"_blank\" rel=\"noreferrer noopener\">disproportionately include seniors, disabled people and children<\/a>, increasing their risk of hunger.\u201d They also say that SNAP cuts would increase the likelihood of Medicaid cuts, particularly the optional programs relied upon by disabled and older people, due to cost-shifting to the states.<\/p>\n<p><strong>Higher costs<\/strong>\u00a0<\/p>\n<p>The Center for Budget and Policy Priorities says Medicaid cuts will also mean higher out-of-pocket costs and more red tape for older adults. This will contribute to poorer health outcomes and loss of benefits, particularly for those ages 50-64, who will face mandatory work requirements. CBPP pointed out that loss of coverage includes low-income people who are working or who should be exempt due to a disability. Over half of such adults, 50 and older, report health as a barrier to employment. CBO estimates that <a href=\"https:\/\/www.cbo.gov\/system\/files\/2025-06\/Wyden-Pallone-Neal_Letter_6-4-25.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">5.2 million adults<\/a> will lose Medicaid under this work requirement, but CBPP said this is likely an underestimate.<\/p>\n<p>Elimination of the <a href=\"https:\/\/acl.gov\/programs\" target=\"_blank\" rel=\"noreferrer noopener\">Administration for Community Living<\/a> and restructuring of its program will also result in significant cuts to older adult services. Here are some of the major changes, <a href=\"https:\/\/www.ncoa.org\/article\/fy26-budget-proposal-puts-aging-services-at-risk\/\" target=\"_blank\" rel=\"noreferrer noopener\">according to the National Council on Aging<\/a>:<\/p>\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/acl.gov\/programs\/health-wellness\/disease-prevention\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Health<\/strong> <strong>Promotion and Disease Prevention<\/strong><\/a> (Preventive Health): Moved to CMS and funding eliminated.<\/li>\n<li><strong>Falls Prevention, Chronic Disease Self-Management Education (CDSME), and Alzheimer\u2019s Disease Initiative<\/strong>: Moved to CMS, but the <a href=\"https:\/\/www.hhs.gov\/open\/prevention\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Prevention and Public Health Fund\u00a0(PPHF) <\/strong><\/a>would be eliminated, along with additional appropriations for falls prevention. The $16.7 million appropriation for Alzheimer\u2019s Disease would be retained.\u00a0This would erase\u00a0a significant source of investment in other public health initiatives as well. The fund not only supports ACL healthy aging grants\u00a0but also key programs at the CDC and the Substance Abuse and Mental Health Services Administration.<\/li>\n<li><a href=\"https:\/\/acl.gov\/programs\/legal-help\/national-center-law-and-elder-rights\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Elder rights<\/strong><\/a><strong>:<\/strong> Moved to CMS and slashed\u00a0to $5 million, eliminating the <strong>Long-Term Care Ombudsman Program, Adult Protective Services <\/strong>support, and other elder abuse, neglect and exploitation efforts.<\/li>\n<li><a href=\"https:\/\/acl.gov\/programs\/aging-and-disability-networks\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Aging Network Support<\/strong><\/a><strong>:<\/strong> Moved to CMS and reduced by over 40% \u2014 potentially eliminating the <strong>Direct Care Workforce Strategies Center<\/strong>.<\/li>\n<li><a href=\"https:\/\/acl.gov\/programs\/aging-and-disability-networks\/aging-and-disability-resource-centers\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Aging and Disability Resource Centers (ADRCs)<\/strong><\/a><strong>:<\/strong> Moved to CMS and eliminated.<\/li>\n<li><strong>Other non-OAA ACL Aging Services: <\/strong>\n<\/li>\n<\/ul>\n<p>The proposed FY26 budget would also eliminate the <a href=\"https:\/\/acf.gov\/ocs\/programs\/liheap\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Low-Income Home Energy Assistance Program<\/strong><\/a> and the <a href=\"https:\/\/acf.gov\/ocs\/programs\/csbg\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Community Services Block Grant<\/strong><\/a><strong> <\/strong>program<strong>. <\/strong>The HHS restructuring would eliminate the <a href=\"https:\/\/www.cdc.gov\/injury\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>CDC\u00a0Injury Center<\/strong><\/a>. No funding would be provided for the CDC\u2019s efforts on <a href=\"https:\/\/www.cdc.gov\/falls\/about\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>falls prevention<\/strong><\/a><strong>,<\/strong> which has included crucial research and data evaluation, and\u00a0efforts like the <a href=\"https:\/\/www.cdc.gov\/steadi\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Stopping Elderly Accidents, Deaths &amp; Injuries (STEADI) Initiative<\/strong><\/a><strong>.<\/strong><\/p>\n<p>Advocacy groups are pushing back against the dismantling of the ACL, and Democrats have launched an inquiry into the move, <a href=\"https:\/\/www.motherjones.com\/politics\/2025\/04\/rfk-jr-bill-community-living\/\" target=\"_blank\" rel=\"noreferrer noopener\">according to Mother Jones<\/a>.
