Then, after assembling it, you discover you\u2019ll be charged even more because you used an Allen wrench.<\/p>\n<p>That\u2019s a standard experience for Americans navigating the health care ecosystem\u2014one that feels deliberately designed to create confusion and drain bank accounts.<\/p>\n<p>The numbers speak for themselves: Americans spend <a href=\"https:\/\/www.healthsystemtracker.org\/chart-collection\/health-spending-u-s-compare-countries\/#Per%20capita%20health%20expenditures,%20U.S.%20dollars,%20PPP%20adjusted,%202021%20and%202022%C2%A0\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">$4,000 more on health care<\/a> every year compared to other high-income countries. <a href=\"https:\/\/newsroom.heart.org\/news\/poll-majority-of-patients-fear-return-of-surprise-medical-bills#:~:text=The%20Harris%20Poll%20conducted%20the,more%20in%20the%20past%20year.\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">Eighteen percent of adults<\/a> received an unexpected medical bill over $1,000 last year. <a href=\"https:\/\/www.kff.org\/medicare\/issue-brief\/what-are-the-consequences-of-health-care-debt-among-older-adults\/#:~:text=Roughly%20two%20in%20five%20Medicare,more%20(data%20not%20shown).\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">One in ten<\/a> seniors have $10,000 or more in medical debt.<\/p>\n<p>But these statistics don\u2019t capture the emotional and psychological toll of committing to care without knowing how much it will cost, then getting blindsided by a bill afterward. The lack of transparency in American health care pricing cannot continue. We have a moral imperative to fix a system that punishes the people it\u2019s meant to protect.<\/p>\n<p><strong>Systemic problems derail transparency.<\/strong><\/p>\n<p>Not knowing what medical procedures or services cost isn\u2019t new. Previous attempts to make pricing clearer have fallen short. The administration recently <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/03\/31\/nx-s1-5342873\/how-much-will-that-surgery-cost-hospital-price-lists-remain-largely-unhelpful?utm_campaign=hcb&amp;utm_medium=newsletter&amp;utm_source=morning_brew\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">signed an executive order<\/a> promising patients more accurate pricing, but the same directive was made six years ago to no avail.<\/p>\n<p>The system feels like it\u2019s designed to benefit anyone but patients because it\u2019s riddled with misaligned incentives that prioritize financial outcomes over quality care.<\/p>\n<p>Most health systems operate under a fee-for-service payment model, which <a href=\"https:\/\/www.commonwealthfund.org\/publications\/explainer\/2023\/feb\/value-based-care-what-it-is-why-its-needed\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">rewards volume over value<\/a>. Providers <a href=\"https:\/\/www.techtarget.com\/healthcarepayers\/feature\/Value-Based-Care-and-Fee-For-Service-Whats-the-Difference\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">are incentivized<\/a> to perform procedures and services with higher profit margins over those that may be more cost-effective for patients. Plus, without price competition, hospitals can increase their margins because <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9636618\/\" data-wpel-link=\"exclude\" target=\"_blank\" rel=\"follow noopener noreferrer nofollow\">patients can\u2019t shop around<\/a>.<\/p>\n<p>Meanwhile, health plans thrive with obscure pricing. They keep negotiated rates with providers confidential and create complex coverage rules to have leverage in provider and patient relationships. The complexity contributes to our nation\u2019s low health insurance literacy; <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8973763\/#:~:text=A%20large%20nationally%20representative%20survey,et%20al.%2C%202014).\" data-wpel-link=\"exclude\" target=\"_blank\" rel=\"follow noopener noreferrer nofollow\">half of Americans<\/a> don\u2019t feel confident when using their health insurance.<\/p>\n<p>These mismatched incentives create a vicious cycle: surprise medical bills stoke mistrust in the health care system. Mistrust leads to care avoidance. Avoiding care worsens conditions, and more severe conditions require more expensive emergency interventions. One survey found <a href=\"https:\/\/www.commonwealthfund.org\/publications\/surveys\/2024\/nov\/state-health-insurance-coverage-us-2024-biennial-survey\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">41 percent of working-age adults<\/a> who delayed care due to cost said their health deteriorated as a result.<\/p>\n<p>These aren\u2019t abstract problems. They create financial devastation and impossible choices for patients during their most vulnerable moments.<\/p>\n<p><strong>It\u2019s not a lost cause: Making transparency work.<\/strong><\/p>\n<p>While previous attempts at reform fell short, we\u2019re now at a critical point where technology, policy, and consumer demand are finally converging to push for real change.<\/p>\n<p><strong>Using technology to create change:<\/strong> Nearly every other industry has improved its consumer experience with modern technologies, and health care needs to catch up. Today\u2019s digital tools make it possible for care teams to quickly share pricing information to help patients understand their coverage and actual costs before care. For instance, a patient can easily confirm that each physical therapy session will cost $125 out-of-pocket and can budget accordingly, avoiding the shock of unexpected bills weeks later. While not widespread yet, these solutions are improving patient experiences while helping save patients money.<\/p>\n<p><strong>Enforcement that matters:<\/strong> Currently, hospitals and health insurance plans face few consequences for not sharing actual care costs, so they don\u2019t make any changes. A <a href=\"https:\/\/oig.hhs.gov\/reports\/all\/2024\/not-all-selected-hospitals-complied-with-the-hospital-price-transparency-rule\/\" data-wpel-link=\"exclude\" target=\"_blank\" rel=\"follow noopener noreferrer nofollow\">recent audit<\/a> found that nearly half (46 percent) of hospitals aren\u2019t following price transparency rules. For transparency to work, we need stronger enforcement, with hospitals and health plans held accountable through significant penalties for non-compliance. Existing policies have too many loopholes and must be reassessed. For example, hospitals shouldn\u2019t be allowed to <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/03\/31\/nx-s1-5342873\/how-much-will-that-surgery-cost-hospital-price-lists-remain-largely-unhelpful?utm_campaign=hcb&amp;utm_medium=newsletter&amp;utm_source=morning_brew\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">report ranges and historical rates<\/a> rather than actual costs, and health insurance plans shouldn\u2019t be allowed to <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/03\/31\/nx-s1-5342873\/how-much-will-that-surgery-cost-hospital-price-lists-remain-largely-unhelpful?utm_campaign=hcb&amp;utm_medium=newsletter&amp;utm_source=morning_brew\" data-wpel-link=\"external\" target=\"_blank\" rel=\"nofollow external noopener noreferrer\">adjust pricing<\/a> based on factors like age without telling patients.<\/p>\n<p><strong>Shared responsibility:<\/strong> Progress requires change from everyone involved. Hospitals, clinics, and insurance companies need to use standardized data so pricing information is consistent and shareable. Insurance companies also need to provide clear, up-to-date cost estimates and offer payment plans so high-cost services don\u2019t delay treatment. Care teams must discuss costs during appointments and tell patients about alternatives. Patients need to use tools that make pricing information more accessible and continue to advocate for appropriate care.
