Here&#8217;s what to know."},"content":{"rendered":"<p id=\"anchor-fb7c97\" class=\"body-graf\">In Minnesota, a family received an <a href=\"https:\/\/minnesotareformer.com\/2025\/05\/06\/the-facility-fee-another-way-hospital-companies-are-squeezing-every-last-nickel-out-of-us\/\" target=\"_blank\" rel=\"nofollow noopener\">unexpected charge of over $400<\/a> after they took their daughter to the doctor for stomach pain. In Ohio, a man was billed <a href=\"https:\/\/www.wsj.com\/health\/healthcare\/hidden-hospital-fees-cost-patients-hundreds-of-dollars-0024cd95?st=4fnenuuid0bm5l3&amp;reflink=desktopwebshare_permalink\" target=\"_blank\" rel=\"nofollow noopener\">$645 extra for an ear, nose and throat specialist<\/a>. In New Hampshire, a resident was charged an additional <a href=\"https:\/\/www.bostonglobe.com\/2024\/10\/22\/metro\/nh-hospital-facility-room-fees-increasing-healthcare-costs-doctor\/?event=event12\" target=\"_blank\" rel=\"nofollow noopener\">$1,000 fee for an appointment with a urologist<\/a>.<\/p>\n<p id=\"anchor-7e24b5\" class=\"body-graf\">Across the country, patients are <a href=\"https:\/\/www.nbcnews.com\/health\/health-care\/cleveland-clinic-florida-patients-facility-fees-rcna219599\" target=\"_blank\" rel=\"nofollow noopener\">expressing frustration<\/a> about \u201cfacility fees\u201d \u2014 charges that a wide range of hospital systems add to bills for appointments at facilities they own, including doctors\u2019 offices offering routine care.<\/p>\n<p id=\"anchor-f420f1\" class=\"body-graf\">A Hospital can charge a facility fees even when a patient hasn\u2019t set foot in a hospital. More than a dozen patients who expected their insurance to cover most of the cost of their appointments at outpatient doctors\u2019 offices told NBC News they were blindsided by the fees, which are billed on top of the cost of seeing medical providers and can easily run into the hundreds of dollars. <\/p>\n<p id=\"anchor-20822f\" class=\"body-graf\"><strong>For more on this story, watch NBC\u2019s \u201cNightly News with Tom Llamas\u201d at 6:30 p.m. ET\/5:30 p.m. CT and \u201cTop Story\u201d on<\/strong><a href=\"https:\/\/www.nbcnews.com\/watch\" target=\"_blank\" rel=\"nofollow noopener\"><strong> NBC News NOW<\/strong><\/a><strong> at 7 p.m. ET. <\/strong><\/p>\n<p id=\"anchor-f79c07\" class=\"body-graf\">The charges have become more prevalent in recent years as <a href=\"https:\/\/www.physiciansadvocacyinstitute.org\/Portals\/0\/assets\/docs\/PAI-Research\/PAI-Avalere%20Physician%20Employment%20Trends%20Study%202019-2023%20Final.pdf?ver=uGHF46u1GSeZgYXMKFyYvw%3d%3d\" target=\"_blank\" rel=\"nofollow noopener\">more physicians are employed by hospitals<\/a> and as <a href=\"https:\/\/www.kff.org\/report-section\/ehbs-2024-summary-of-findings\/\" target=\"_blank\" rel=\"nofollow noopener\">insurance plans leave patients paying more<\/a> for care before their coverage kicks in. In some communities, there is<a href=\"http:\/\/kff.org\/health-costs\/issue-brief\/ten-things-to-know-about-consolidation-in-health-care-provider-markets\/\" target=\"_blank\" rel=\"nofollow noopener\"> so much health care consolidation<\/a> that it\u2019s hard to find practices that don\u2019t charge facility fees. <\/p>\n<p id=\"anchor-701919\" class=\"body-graf\">Researchers say patchwork laws to regulate the fees haven\u2019t kept up. <\/p>\n<p id=\"anchor-a93191\" class=\"body-graf\">\u201cIn most states and situations, there aren\u2019t really limits on how high they can go,\u201d said Christine Monahan, an assistant research professor at the Center on Health Insurance Reforms at Georgetown University, who has studied facility fees.<\/p>\n<p id=\"anchor-4c832d\" class=\"body-graf\">Hospitals argue facility fees are necessary to fund the higher level of care they say they provide at their outpatient doctors\u2019 offices, as well as to help maintain 24\/7 services such as emergency rooms. The American Hospital Association says facility fees should be covered by insurance companies, while insurers say the fees unnecessarily inflate the cost of care without improving its quality. <\/p>\n<p id=\"anchor-4dff5e\" class=\"body-graf\">Here\u2019s what consumer advocates and health policy experts say patients should know about facility fees.<\/p>\n<p><strong>Before your appointment: What to ask<\/strong><\/p>\n<p id=\"anchor-1fe9f1\" class=\"body-graf\">Experts recommend you ask every time you book an appointment whether there is a facility fee \u2014 even for physicians you have seen before. A doctor\u2019s office ownership, hospital affiliation or policies may have changed since your last visit. <\/p>\n<p id=\"anchor-9f1c26\" class=\"body-graf\">If you are told there will be a facility fee, ask for a good-faith estimate of what the anticipated charge will be, said Patricia Kelmar, senior director of health care campaigns at the U.S. Public Interest Research Group, a consumer advocacy organization.<\/p>\n<p id=\"anchor-b5202c\" class=\"body-graf\">And don\u2019t assume your insurance will cover the bill.  <\/p>\n<p id=\"anchor-715c65\" class=\"body-graf\">Last year, Melissa Finnegan, of St. Paul, Minnesota, was charged a facility fee of $423.15 for an appointment with a pediatric gastroenterologist for her 3-year-old. When she tried to fight it, she discovered neither the health system nor her insurance company was willing to budge. <\/p>\n<p id=\"anchor-4dc890\" class=\"body-graf\">\u201cI put off paying it for as long as I could,\u201d she said.<\/p>\n<p><strong>When you get the bill: How to make sense of the charges<\/strong><\/p>\n<p id=\"anchor-be45c3\" class=\"body-graf\">If you receive an unexpectedly large medical bill after an appointment, call the billing office to ask for an itemized bill to understand the charges. Outpatient facility fees may be listed under other names, such as a \u201cclinic\u201d fee. In office signs and other disclosures about facility fees, hospitals may refer to the practice as \u201cprovider-based billing,\u201d which means patients\u2019 bills can be split into two separate charges: one for the physician\u2019s services and one for visiting the facility.<\/p>\n<p id=\"anchor-f4a4f5\" class=\"body-graf\">\u201cPeople see the bill and just assume, because it\u2019s on their bill, that they\u2019re going to have to pay it,\u201d said Eric Waskowicz, senior state policy manager at United States of Care, a nonpartisan organization that fights for affordable health care.<\/p>\n<p id=\"anchor-dff28a\" class=\"body-graf\">But in some cases, you may not have to pay. Insurers suggest patients wait until they receive their explanation of benefits so they can review what part their plans will cover and they don\u2019t accidentally overpay.<\/p>\n<p id=\"anchor-e02bad\" class=\"body-graf\">You should also check <a href=\"https:\/\/unitedstatesofcare.org\/wp-content\/uploads\/2023\/06\/State-Successes-Passing-Laws-to-Promote-Fair-Billing_Facility-Fees.pdf\" target=\"_blank\" rel=\"nofollow noopener\">the laws in your state<\/a> to make sure you were correctly charged: <a href=\"https:\/\/facilityfeereform.chir.georgetown.edu\/facility-fee-prohibitions\/\" target=\"_blank\" rel=\"nofollow noopener\">Some states ban facility fees<\/a> for telehealth visits, for example, while others prohibit facility fees for preventive services or at certain types of medical offices.<\/p>\n<p id=\"anchor-ad9d02\" class=\"body-graf\">\u201cKnow what the law is, and then use it if you can to protect yourself from that fee,\u201d Kelmar said.<\/p>\n<p id=\"anchor-34e1db\" class=\"body-graf\">The American Hospital Association advises patients to familiarize themselves with their insurance plans\u2019 coverage and says that if they\u2019ve received unexpected facility fee charges to confirm that their insurance has \u201cappropriately processed\u201d their claims. <\/p>\n<p><strong>What to do if you can\u2019t afford to pay<\/strong><\/p>\n<p id=\"anchor-117efe\" class=\"body-graf\">If you can\u2019t afford to pay a facility fee, try calling the medical provider\u2019s office to see whether the charge can be removed. If that doesn\u2019t work, talk to your insurance company, Kelmar said. Insurance may be able to work with the billing office to reduce your out-of-pocket responsibility.  <\/p>\n<p id=\"anchor-313e95\" class=\"body-graf\">There are other options for those who can\u2019t pay. Sometimes billing departments will give discounts to people who offer to pay smaller amounts immediately, rather than pay the larger amounts over time.  <\/p>\n<p id=\"anchor-1b676b\" class=\"body-graf\">Patients can also ask about paying the cash price, instead, meaning the charge they would have incurred if they didn\u2019t have insurance, said Monahan, the Georgetown expert. The <a href=\"https:\/\/www.forbes.com\/sites\/gebai\/2024\/04\/21\/why-are-cash-prices-lower-than-health-insurance-negotiated-prices\/\" target=\"_blank\" rel=\"nofollow noopener\">cash price might be lower<\/a>. <\/p>\n<p id=\"anchor-02d2f8\" class=\"body-graf\">If that doesn\u2019t work, <a href=\"https:\/\/www.cms.gov\/medical-bill-rights\/help\/guides\/patient-advocate\" target=\"_blank\" rel=\"nofollow noopener\">search for a health care advocate<\/a> in your state, Waskowicz suggested. <\/p>\n<p id=\"anchor-be627a\" class=\"body-graf\">\u201cTo the extent it\u2019s possible, they are able to work with insurance providers to see if they can get that fee waived,\u201d he said.<\/p>\n<p id=\"anchor-05f7ae\" class=\"body-graf\">In the meantime, keep in touch with the billing office, Waskowicz added. If you are actively contesting the bill, chances are lower that it will be sent to collections.<\/p>\n<p id=\"anchor-131f18\" class=\"body-graf\">Kelmar said that if you can\u2019t avoid a facility fee and are struggling to pay, don\u2019t put the balance on a credit card. Instead, work out a payment plan directly with the provider, ideally with low or no interest.<\/p>\n<p><strong>What to do ahead of your next appointment <\/strong><\/p>\n<p id=\"anchor-f26a8a\" class=\"body-graf\">If you want to avoid facility fees in the future, try to find an independent doctor\u2019s office that isn\u2019t owned by a hospital. <\/p>\n<p id=\"anchor-9dbf81\" class=\"body-graf\">That isn\u2019t always easy: In West Covina, California, Todd Bash, 60, needed injections for a spinal problem and ended up being billed facility charges of over $450 out of pocket for a pain specialist. He then made dozens of calls trying to find a different physician who could administer the injections. But he said all the doctors\u2019 offices he reached out to were affiliated with hospitals or didn\u2019t take his insurance.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.europesays.com\/us\/wp-content\/uploads\/2025\/07\/250721-todd-bash-facility-fees-inline-lr-6de8c4.jpg\" alt=\"Todd Bash has had trouble finding a medical provider who doesn't charge facility fees.\" height=\"3024\" width=\"4032\"\/>Todd Bash has had trouble finding a medical provider who doesn&#8217;t charge facility fees.Aarne Heikkila \/ NBC News <\/p>\n<p id=\"anchor-50d6d1\" class=\"body-graf\">\u201cIt was like a full-time job trying to just navigate the system and trying to get a price,\u201d he said. <\/p>\n<p id=\"anchor-6a08eb\" class=\"body-graf\">Bash finally found an independent clinic about 20 miles away that accepted his insurance and would administer the shots for $37. Then, last month, the doctor\u2019s office stopped accepting insurance.<\/p>\n<p id=\"anchor-60b19e\" class=\"body-graf\">\u201cI\u2019m back to square one,\u201d he said.<\/p>\n<p id=\"anchor-9b7e57\" class=\"body-graf\">California doesn\u2019t have laws on the books that address facility fees.
