(AP) \u2014 The U.S. Senate voted Friday to confirm Mark Cruz, a citizen of the Klamath Tribes, to lead the federal agency that provides healthcare for some 2.8 million Native American and Alaska Native patients. <\/p>\n<p>Cruz was confirmed to serve a four-year term as director of the Indian Health Service, a position that has sat vacant since late 2025. <\/p>\n<p>At his July confirmation hearing, Cruz said he would advocate \u201cferociously\u201d to improve health outcomes in Indian Country and bring lived experience to the role. <\/p>\n<p>\u201cMy family, my friends and my tribal community will live with the consequences of the work I do for decades to come,\u201d Cruz told the Senate Indian Affairs Committee. <\/p>\n<p>Cruz was confirmed by a 51-47 vote along with <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/fema-cameron-hamilton-trump-disaster-aid-reforms-6743eb69461fc625d924396842056bba\" rel=\"nofollow noopener\" target=\"_blank\">more than 70 other nominees<\/a> of President Donald Trump \u2014 one of the Senate\u2019s last actions before taking an August recess. <\/p>\n<p>Cruz is a senior adviser on tribal health to <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/healthy-tribes-rfk-chronic-disease-native-american-1b7f0a2da601fb54cd8cfa0a7a3c4b0c\" rel=\"nofollow noopener\" target=\"_blank\">Health Secretary Robert F. Kennedy Jr.<\/a> and will retain that newly created role while leading the IHS. In Trump\u2019s first administration, Cruz oversaw the Office of Indian Energy and Economic Development under the U.S. Bureau of Indian Affairs. <\/p>\n<p>The IHS operates 21 hospitals and 78 smaller health centers nationwide as part of the federal government\u2019s legal responsibility to provide healthcare for Native Americans. The average age of those facilities is around 40 years old and one-third are in \u201cpoor\u201d physical condition, according to a 2023 U.S. Government Accountability Office report.<\/p>\n<p>Cruz assumes leadership of an agency grappling with stubbornly high provider vacancy rates and a <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/indian-health-service-construction-backlog-1993-native-american-b779a94f5885113da847d997460b1623\" rel=\"nofollow noopener\" target=\"_blank\">30-year backlog<\/a> of facilities in need of replacement. He has pledged to address those challenges, <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/government-shutdown-native-americans-services-e70167fd8306097709bddfa7ded55474\" rel=\"nofollow noopener\" target=\"_blank\">better insulate<\/a> tribal health programs from federal budget impasses and fund <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/native-american-tribes-healthcare-indian-health-service-2b13ca21d150ab3ee1d85c966f387d54\" rel=\"nofollow noopener\" target=\"_blank\">overhead costs<\/a> tribes take on when they manage their own healthcare systems. <\/p>\n<p>In a statement Friday, the National Congress of American Indians said Cruz\u2019s confirmation comes at a \u201ccritical time.\u201d <\/p>\n<p>\u201cWe congratulate him on his confirmation and look forward to working together to ensure the federal government fully upholds its <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/trump-tribal-nations-bureau-of-indian-affairs-f546838f7d480c99cdbdf8ab0f3b5859\" rel=\"nofollow noopener\" target=\"_blank\">trust and treaty obligations<\/a>,\u201d NCAI executive director Larry Wright Jr. said.<\/p>\n<p>The National Indian Health Board also applauded Cruz\u2019s confirmation. By that nonprofit\u2019s most recent estimate, it would take some $73 billion for the IHS to fully meet the healthcare needs of tribal nations.
